What Is a Maximum Performance Test?
A maximum performance test (German: Ausleistungstest) is a diagnostic or technical procedure in which a system, device, or organism is subjected to progressively increasing demands until its maximum capacity is reached. The test yields important data on performance limits, load tolerance, and functional efficiency.
In medicine, the term is most commonly used in the context of performance diagnostics, particularly in sports medicine, cardiac rehabilitation, and pulmonology. The goal is to objectively determine individual performance levels and derive targeted recommendations based on the results.
Fields of Application
Sports Medicine and Performance Diagnostics
In sports medicine, the maximum performance test is used to determine maximal oxygen uptake (VO2max), the anaerobic threshold, and muscular endurance. Common test formats include treadmill tests, bicycle ergometer tests, and step tests with progressively increasing workloads.
Cardiology and Rehabilitation
In cardiology, the maximum performance test assesses cardiac function under full physical load. Heart rate, blood pressure, and ECG changes are continuously monitored. The test helps identify conditions such as coronary artery disease or cardiac arrhythmias, and is also used to evaluate rehabilitation progress following a myocardial infarction.
Pulmonology
In respiratory medicine, the maximum performance test evaluates respiratory tolerance in patients with conditions such as bronchial asthma, COPD, or interstitial lung diseases. Parameters measured include lung volume, oxygen saturation, and exercise tolerance.
Medical Technology
The term is also applied in the field of medical technology, where devices and systems are subjected to maximum performance testing to ensure reliable and safe operation under peak demand. This is especially critical for life-sustaining systems such as ventilators or cardiac pacemakers.
Procedure
A maximum performance test is typically conducted under controlled conditions and medical supervision. The workload is gradually increased until either maximum performance is achieved or predefined termination criteria are met. Common termination criteria include:
- Exhaustion of the test subject
- Critical ECG changes
- Significant rise or drop in blood pressure
- Onset of pain or severe breathlessness
Evaluation and Clinical Relevance
The data obtained from a maximum performance test enable a precise assessment of individual performance reserves. They serve as the basis for:
- Creating personalized training or therapy plans
- Monitoring the progress of chronic conditions
- Evaluating fitness for work or sport
- Preventing overexertion injuries
By comparing results with reference values adjusted for age, sex, and body weight, individual strengths and weaknesses can be assessed objectively.
Risks and Contraindications
Because a maximum performance test intentionally pushes a person to their limits, it is not suitable for all individuals. Contraindications may include:

- Uncontrolled high blood pressure
- Severe valvular heart disease
- Acute infectious illness
