What Is Sedimentation?
Sedimentation in medicine describes the process by which solid particles – primarily red blood cells (erythrocytes) – settle to the bottom of a standing liquid under the influence of gravity. In clinical practice, this is most commonly measured as the erythrocyte sedimentation rate (ESR), also known as the blood sedimentation rate. It is one of the oldest and most widely used laboratory tests in medicine.
How the Test Works
To measure the ESR, a blood sample is mixed with an anticoagulant to prevent clotting and placed in a vertical, graduated tube. Gravity causes the erythrocytes to gradually sink. The distance they have settled is measured in millimeters after one hour and optionally after two hours.
Factors That Influence Sedimentation
- Inflammatory proteins: Acute-phase proteins such as fibrinogen cause red blood cells to stack together in formations called rouleaux, which increases their rate of settling.
- Age and sex: Women and older individuals generally have slightly higher normal values.
- Anemia: A reduced red blood cell count can raise the ESR.
- Pregnancy: A physiologically elevated ESR is normal during pregnancy.
Normal Reference Values
The reference ranges for ESR vary by age and sex:
- Men under 50 years: up to 15 mm/h
- Men over 50 years: up to 20 mm/h
- Women under 50 years: up to 20 mm/h
- Women over 50 years: up to 30 mm/h
Values significantly above these reference ranges may indicate an inflammatory, infectious, or oncological condition.
Clinical Significance
The ESR is a non-specific marker of inflammation. This means it signals that an inflammatory process is occurring in the body but does not indicate its precise cause. Elevated ESR values can be found in the following conditions:
- Bacterial and viral infections
- Autoimmune diseases (e.g., rheumatoid arthritis, systemic lupus erythematosus)
- Inflammatory bowel diseases (e.g., Crohn's disease, ulcerative colitis)
- Malignancies (e.g., multiple myeloma, lymphoma)
- Vasculitis, particularly polymyalgia rheumatica and giant cell arteritis
Low Sedimentation Values
A low ESR is rarely clinically significant but may occur in polycythemia (an elevated red blood cell count), severe heart failure, or certain hemoglobinopathies.
ESR Compared to Other Inflammatory Markers
The ESR is often measured alongside C-reactive protein (CRP). CRP responds more rapidly to acute inflammation and rises within hours, whereas the ESR reacts more slowly but remains elevated for a longer period. Both markers complement each other in clinical diagnostics.
Limitations of the Test
Because sedimentation is a non-specific parameter, it cannot be used as a standalone diagnostic tool. Accurate diagnosis always requires additional clinical examination, specific laboratory values, and, where appropriate, imaging studies. Nevertheless, the ESR remains a valuable screening and monitoring tool in clinical practice.
