What is Semax?
Semax is a synthetic peptide originally developed in the Soviet Union and Russia, structurally derived from adrenocorticotropic hormone (ACTH). Its amino acid sequence is Met-Glu-His-Phe-Pro-Gly-Pro. Semax belongs to the class of nootropics – substances intended to enhance cognitive performance. It is approved as a prescription drug in Russia and Ukraine, while in most other countries it is classified as a research substance.
Mechanism of Action
Semax exerts its effects through several biological pathways:
- Increase in BDNF levels: Semax stimulates the release of Brain-Derived Neurotrophic Factor (BDNF), a key growth factor for neurons that supports their survival, growth, and plasticity.
- Modulation of dopaminergic and serotonergic systems: Semax influences dopamine and serotonin activity in the brain, which may explain its mood-enhancing and attention-boosting effects.
- Anti-inflammatory effects in the CNS: Semax demonstrates anti-inflammatory properties within the central nervous system (CNS), contributing to its neuroprotective profile.
- Improvement of cerebral blood flow: Semax may enhance microcirculation in the brain and improve oxygen supply to nerve cells.
Medical Applications
In Russia and Ukraine, Semax is used clinically in the following areas:
- Treatment of ischemic stroke and post-stroke rehabilitation

- Neurodegeneration and neuroprotection following traumatic brain injury

Administration and Dosage
Semax is most commonly administered as a nasal spray (intranasal application), as this route allows it to reach the brain directly via the nasal mucosa. It is also available in injectable form. Typical dosages used in clinical studies range from 200 to 900 micrograms per day, divided into multiple administrations. The exact dosage should always be determined by a qualified healthcare professional.
Side Effects
Semax is generally considered to be well tolerated. Possible side effects include:
- Irritation of the nasal mucosa with intranasal use

- Temporary mood fluctuations

Scientific Evidence
The majority of clinical research on Semax originates from Russia and Ukraine. Independent international studies remain scarce. Preclinical studies (animal models) show promising neuroprotective and cognitive-enhancing effects; however, the transferability of these results to humans has not yet been fully established. In Western countries, Semax is therefore primarily used in the context of self-biohacking or as a research chemical.
