What is the Pelvic Floor?
The pelvic floor is a group of muscles, ligaments, and connective tissue forming the base of the pelvis. It supports the pelvic organs — including the bladder, bowel, and in women, the uterus — and plays a crucial role in bladder and bowel control as well as sexual function. A well-trained pelvic floor significantly contributes to quality of life.
Why is Pelvic Floor Training Important?
A weakened pelvic floor can lead to a variety of health problems. Women after pregnancy and childbirth are particularly affected, but men — for example after prostate surgery — and older adults can also benefit greatly from targeted pelvic floor training.
- Urinary incontinence: involuntary urine leakage when coughing, sneezing, or laughing
- Fecal incontinence: lack of bowel control
- Pelvic organ prolapse: descent of the bladder, uterus, or bowel
- Lower back pain: a weak pelvic floor can place strain on the lumbar spine
- Sexual dysfunction: reduced sensation or pain during intercourse
Causes of Pelvic Floor Weakness
The pelvic floor can be weakened by several factors:
- Pregnancy and vaginal childbirth
- Overweight and persistently elevated abdominal pressure
- Chronic coughing (e.g., in smokers)
- Hormonal changes during menopause
- Pelvic surgery
- Physical inactivity and prolonged sitting
Exercises to Strengthen the Pelvic Floor
Kegel Exercises
Kegel exercises are the most well-known method for targeted pelvic floor strengthening. The pelvic floor muscles are contracted as if stopping the flow of urine, held for a few seconds, and then consciously released. This exercise should be repeated several times daily.
Bridging
Lying on the back with feet flat on the floor, the pelvis is slowly lifted while engaging the pelvic floor and held for a few seconds. This exercise also strengthens the gluteal and back muscles simultaneously.
Deep Squats
Controlled deep squats activate the pelvic floor together with the entire core musculature and promote functional strength for everyday activities.
Yoga and Pilates
Yoga and Pilates incorporate many exercises that indirectly engage the pelvic floor and improve body awareness of this muscle group. They are particularly well suited for beginners.
Pelvic Floor Training for Men
Men also have a pelvic floor that can and should be trained. Especially after prostate surgery or in cases of erectile dysfunction, targeted pelvic floor training can improve continence and support sexual function. The exercises are similar to those performed by women and can be done discreetly during everyday activities.
Professional Support and Physiotherapy
For significant symptoms, working with a specialized pelvic floor physiotherapist is highly recommended. Techniques such as biofeedback can improve awareness and coordination of the pelvic floor muscles. Electrical stimulation is also used therapeutically to promote muscle activity. Medical consultation is especially important when incontinence or pain is present.
