What is Thyroid Hormone Therapy?
Thyroid hormone therapy is a medical treatment in which thyroid hormones are administered externally to compensate for a deficiency in the body. The thyroid gland is a butterfly-shaped gland located at the front of the neck that produces essential hormones, primarily thyroxine (T4) and triiodothyronine (T3). These hormones regulate metabolism, body temperature, cardiovascular function, and growth and development.
When the thyroid gland does not produce sufficient hormones (hypothyroidism) or after surgical removal of the thyroid, the body must receive these hormones through medication.
Indications – When is the Therapy Used?
Thyroid hormone therapy is used in a variety of medical situations:
- Hypothyroidism: The most common indication. The thyroid produces too few hormones, leading to fatigue, weight gain, and low energy.
- Hashimoto Thyroiditis: An autoimmune disease in which the immune system attacks the thyroid gland, progressively impairing its function.
- After thyroid surgery (thyroidectomy): Following total or partial removal of the thyroid, hormone production must be fully replaced.
- After radioiodine therapy: Radioiodine treatment can permanently reduce thyroid function, requiring hormonal supplementation.
- Thyroid cancer (postoperative): Higher-dose therapy is often used to suppress the growth of any remaining tumor cells.
- Goiter (enlarged thyroid): In some cases, used to help reduce the size of an enlarged thyroid gland.
Mechanism of Action
The most commonly used medication is levothyroxine (L-thyroxine), a synthetic form of the naturally occurring T4 hormone. After ingestion, levothyroxine is converted in the body by the enzyme deiodinase into the biologically active T3 (triiodothyronine). T3 binds to receptors in nearly every cell in the body, influencing gene expression and a wide range of metabolic processes.
This therapy mimics the natural function of the thyroid gland and restores the hormonal balance known as euthyroidism.
Dosage and Administration
The dosage of thyroid hormone therapy is individualized and determined by the physician based on blood tests, particularly the TSH (thyroid-stimulating hormone) level. It is regularly adjusted as needed. General guidelines for taking the medication include:
- Levothyroxine is typically taken in the morning on an empty stomach, approximately 30 minutes before breakfast.
- The tablet should be swallowed with a full glass of water.
- Certain foods such as soy products, coffee, and high-fiber foods, as well as medications like calcium supplements or antacids, can impair hormone absorption and should be taken at a different time of day.
- The medication should be taken consistently and without self-adjusting the dose.
Side Effects
When correctly dosed, thyroid hormone therapy is generally well tolerated. Side effects occur mainly with overdosing and can cause symptoms of hyperthyroidism (overactive thyroid):
- Rapid or irregular heartbeat (palpitations or arrhythmias)
- Nervousness, restlessness, and sleep disturbances
- Excessive sweating and heat intolerance
- Weight loss and increased appetite
- Trembling (tremor)
- Diarrhea or more frequent bowel movements
If any of these symptoms occur, a physician should be consulted promptly, as a dose adjustment may be necessary.
Course of Treatment and Prognosis
For most patients, thyroid hormone therapy is a lifelong treatment, especially when the underlying cause of hypothyroidism is permanent (e.g., after thyroidectomy or in Hashimoto thyroiditis). Regular follow-up appointments are essential to adjust the dosage and maintain hormone levels within the optimal range. With well-managed therapy, patients can lead a completely normal and symptom-free life.
Special Patient Groups
Pregnant Women
During pregnancy, the need for thyroid hormones often increases, as the mother must also supply hormones for the developing baby. Close monitoring and possible dose adjustments are particularly important, as a hormone deficiency can increase the risk of miscarriage and developmental disorders in the child.
Elderly Patients
In older adults and patients with heart conditions, therapy is usually started at a lower dose and increased gradually to avoid placing excessive strain on the cardiovascular system.
Children and Adolescents
For children with congenital hypothyroidism, early and consistent hormone therapy is essential for normal mental and physical development and prevention of cretinism.
