What Is Bile Acid Therapy?
Bile acid therapy refers to the medical use of natural or synthetically produced bile acids to treat various diseases of the liver, bile ducts, and gallbladder. Bile acids are endogenous substances produced in the liver from cholesterol. They play a central role in fat digestion and the excretion of cholesterol. The most well-known therapeutically used bile acids are ursodeoxycholic acid (UDCA) and obeticholic acid (OCA).
Indications
Bile acid therapy is used across a range of conditions:
- Cholesterol gallstones: UDCA can dissolve small to medium-sized pure cholesterol gallstones (litholysis) when surgery is not possible or desired.
- Primary biliary cholangitis (PBC): A chronic, cholestatic liver disease for which UDCA is the standard therapy, helping to slow disease progression.
- Primary sclerosing cholangitis (PSC): An inflammatory disease of the bile ducts in which bile acids may be used as supportive treatment.
- Intrahepatic cholestasis of pregnancy: UDCA relieves itching and improves liver function tests in this pregnancy-specific condition.
- Non-alcoholic fatty liver disease (NAFLD/NASH): Newer agents such as obeticholic acid are being investigated in clinical trials for their efficacy in fatty liver disease.
Mechanism of Action
Bile acids exert their therapeutic effects through several mechanisms:
- Choleretic effect: They stimulate bile production and bile flow, helping to flush stagnant or toxic bile from the liver.
- Displacement and solubilization: UDCA displaces toxic, hydrophobic bile acids and replaces them with less harmful, hydrophilic variants, protecting liver cells from damage.
- Immunomodulation: Bile acids can suppress inflammatory processes in the bile ducts and liver.
- Gallstone dissolution: By reducing cholesterol saturation in bile, UDCA can gradually dissolve cholesterol gallstones.
- FXR agonism: Obeticholic acid activates the farnesoid X receptor (FXR), a nuclear receptor that regulates bile acid synthesis, lipid metabolism, and inflammatory processes.
Dosage and Administration
The dosage of bile acid therapy depends on the underlying condition, body weight, and individual patient response. For UDCA in primary biliary cholangitis, the standard recommendation is 13–15 mg per kilogram of body weight per day, divided into multiple doses. In gallstone dissolution therapy, treatment may last from several months up to two years. Therapy is prescribed and monitored exclusively by a physician, with regular liver function testing.
Side Effects and Tolerability
UDCA is generally well tolerated. Possible side effects include:
- Diarrhea or loose stools
- Nausea or gastrointestinal discomfort
- Rarely: itching or skin rash
Obeticholic acid can cause intensified itching (pruritus) at higher doses, making careful dose adjustment necessary. Both agents are generally not suitable for patients with decompensated liver cirrhosis.
Diagnosis and Monitoring
Before initiating bile acid therapy, laboratory tests including liver enzymes (ALT, AST, GGT, alkaline phosphatase), bilirubin levels, and imaging procedures such as ultrasound are typically performed. Regular follow-up examinations during therapy are essential to assess treatment response and detect potential side effects early.
