What Are Breathing Difficulties?
Breathing difficulties is an umbrella term for any form of labored, painful, or uncomfortable breathing. Medically, several presentations are distinguished, including dyspnea (the subjective sensation of breathlessness), tachypnea (abnormally rapid breathing), and orthopnea (breathlessness when lying flat). Breathing difficulties can occur both acutely and chronically and affect people of all ages.
Causes
The causes of breathing difficulties are diverse and commonly involve the airways, lungs, cardiovascular system, or psychological factors.
Common Respiratory and Pulmonary Causes
- Asthma: a chronic inflammatory airway disease causing episodes of breathlessness, wheezing, and chest tightness
- Chronic Obstructive Pulmonary Disease (COPD): progressive narrowing of the airways, often caused by smoking
- Pneumonia: infection of lung tissue by bacteria, viruses, or fungi
- Pulmonary embolism: blockage of a pulmonary blood vessel by a blood clot
- Pneumothorax: air entering the chest cavity causing a lung to collapse
- Acute bronchitis: inflammation of the bronchial tubes, often triggered by viral infections
Cardiac Causes
- Heart failure: reduced pumping capacity of the heart leading to fluid accumulation in the lungs
- Cardiac arrhythmias: irregular heartbeat that can impair oxygen supply to the body
- Heart attack (myocardial infarction): acute blockage of a coronary artery, which may present with breathlessness
Other Causes
- Allergic reactions or anaphylactic shock
- Anemia: reduced oxygen-carrying capacity of the blood
- Obesity: mechanical restriction of respiratory function
- Psychological causes: panic attacks and anxiety disorders can trigger episodes of breathlessness
- Foreign body obstruction in the airways (especially in children)
Symptoms
Breathing difficulties can manifest in various forms. Typical accompanying signs and presentations include:
- A feeling of breathlessness or suffocation
- Wheezing or crackling breathing sounds (stridor or wheezing)
- Pain when breathing (pleuritis or muscular pain)
- Shortness of breath during physical activity or at rest
- Bluish discoloration of the lips or fingernails (cyanosis), indicating oxygen deficiency
- Cough, sputum production, or coughing up blood
- Rapid heartbeat (tachycardia) as an accompanying symptom
When to Seek Medical Help?
Certain situations require immediate medical attention. Call emergency services or go to the nearest emergency department immediately if:
- sudden severe breathlessness occurs
- breathing difficulties are accompanied by chest pain, loss of consciousness, or cyanosis
- breathing problems develop after an accident or injury
- children show visible chest retractions with breathing difficulties
Persistent or worsening breathing difficulties without a clear cause should also be evaluated by a doctor promptly.
Diagnosis
To diagnose breathing difficulties, physicians use a range of examination methods:
- Medical history and physical examination: questions about onset, duration, and circumstances of symptoms, along with auscultation of the lungs
- Pulmonary function testing (spirometry): measurement of lung capacity and detection of obstructions
- Blood gas analysis: measurement of oxygen and carbon dioxide levels in the blood

- ECG and echocardiography: to evaluate cardiac function
- Allergy and laboratory tests: to rule out allergic or metabolic causes
Treatment
Treatment of breathing difficulties is always directed at the underlying cause.
Medical Treatment
- Bronchodilators (e.g., salbutamol) for asthma or COPD to widen the airways
- Corticosteroids to reduce inflammation in asthma or severe bronchitis
- Antibiotics for bacterial respiratory infections
- Diuretics for breathlessness caused by heart failure to remove excess fluid
Non-Medical Measures
- Breathing therapy and physiotherapy to improve breathing technique
- Oxygen therapy in cases of severe oxygen deficiency
- Weight reduction for breathlessness related to obesity
- Smoking cessation for the prevention and treatment of COPD
