What Are Lymphocyte Activation Markers?
Lymphocyte activation markers are specific molecules, typically proteins expressed on the cell surface of lymphocytes, that appear or increase in number when these immune cells are triggered by a stimulus such as an antigen, pathogen, or cytokine. Lymphocytes are white blood cells that play a central role in the adaptive immune system. The main types are T lymphocytes, B lymphocytes, and natural killer (NK) cells.
Activation markers serve as measurable indicators of the immune system state. In clinical diagnostics, they are used to assess immune responses, identify immune deficiencies, and monitor the progression of diseases or the effectiveness of therapies.

CD25 (IL-2 Receptor Alpha)
CD25 is one of the best-known activation markers. It is upregulated on T lymphocytes following antigen contact and forms part of the interleukin-2 (IL-2) receptor complex. Elevated CD25 expression indicates an active T cell response. It is also constitutively expressed on regulatory T cells (Tregs).
CD69
CD69 is an early activation marker that appears on T cells, B cells, and NK cells within just a few hours of stimulation. It is considered a sensitive early indicator of immune activation and is widely used in laboratory testing.
CD71 (Transferrin Receptor)
CD71 is expressed on proliferating cells and indicates that lymphocytes are in an active phase of division. It serves as a marker of advanced or sustained activation.
HLA-DR
HLA-DR is an MHC class II molecule expressed on activated T cells (particularly CD8-positive T cells) and B cells. Elevated HLA-DR expression on CD8 T cells is characteristic of chronic viral infections or autoimmune responses.
CD38
CD38 is frequently assessed together with HLA-DR to evaluate T cell activation. Co-expression of both markers on CD8 T cells is an important indicator of active infection, for example in HIV disease.
Clinical Significance
The measurement of lymphocyte activation markers is of great importance across several medical fields:
- Infectious diseases: Viral infections such as HIV, Epstein-Barr virus (EBV), or cytomegalovirus (CMV) lead to characteristic upregulation of activation markers on lymphocytes.
- Autoimmune diseases: In conditions such as systemic lupus erythematosus (SLE) or rheumatoid arthritis, lymphocytes are persistently activated, resulting in elevated marker levels.
- Immune deficiencies: Primary or secondary immunodeficiencies may be associated with reduced or abnormal expression of activation markers.
- Transplant medicine: After organ or stem cell transplantation, activation markers are monitored to detect rejection reactions at an early stage.
- Oncology: In cancer therapy, especially with immunotherapies such as checkpoint inhibitors or CAR-T cell therapy, activation markers serve as parameters to assess treatment response.
Diagnostic Methods
The most common method for measuring lymphocyte activation markers is flow cytometry (also known as FACS analysis). In this technique, blood cells are stained with fluorescently labeled antibodies targeting specific surface markers and then analyzed in a flow cytometer. This allows simultaneous identification of lymphocyte type (T, B, or NK cells) and their activation status.
Additional methods include immunohistochemistry on tissue sections and molecular biology techniques such as quantitative PCR to measure the gene expression of activation markers.
