What is Loratadine?
Loratadine is a second-generation antihistamine that works by selectively blocking H1 histamine receptors. Unlike older, first-generation antihistamines, loratadine has a much lower sedating effect because it does not readily cross the blood-brain barrier.

When the body is exposed to an allergen, mast cells release histamine, which binds to H1 receptors on various cells and causes symptoms such as itching, sneezing, swelling, and redness. Loratadine competitively blocks these H1 receptors, preventing histamine from exerting its effects. Because loratadine crosses the blood-brain barrier only minimally, it causes significantly less drowsiness compared to first-generation antihistamines such as diphenhydramine.
Dosage and Administration
The standard recommended dose for adults and children aged 12 years and older is 10 mg loratadine once daily. Loratadine is available in several forms:
- Tablets (10 mg)
- Orally disintegrating tablets
- Syrup (for children)
For children between 2 and 12 years of age, dosing is based on body weight. Loratadine can be taken with or without food. Dosage adjustment may be necessary in patients with significant liver impairment. Always follow the instructions on the package leaflet or consult a healthcare professional.
Side Effects
Loratadine is generally well tolerated. Possible side effects include:
- Common: headache, dry mouth, fatigue (usually mild and infrequent)
- Occasional: nausea, stomach discomfort, nervousness
- Rare: cardiac arrhythmias, elevated liver enzymes, hypersensitivity reactions to the active substance
Compared to first-generation antihistamines, the risk of significant drowsiness is very low, making loratadine suitable for people who drive or operate machinery.
Drug Interactions
Certain medications may increase blood levels of loratadine, including:
- Macrolide antibiotics (e.g., erythromycin)
- Antifungals (e.g., ketoconazole)
- Cimetidine (a stomach acid blocker)
These interactions are generally not clinically significant given the wide therapeutic window of loratadine. However, patients taking multiple medications should always consult a doctor or pharmacist.
Contraindications and Precautions
Loratadine should not be used in cases of:
- Known hypersensitivity to loratadine or any of its excipients
