What Is Lactate Measurement?
Lactate measurement is a diagnostic procedure used to determine the concentration of lactate (the salt of lactic acid) in the blood. Lactate is produced as a metabolic byproduct when cells generate energy without sufficient oxygen – a process known as anaerobic glycolysis. The measurement provides essential information about tissue oxygenation, cardiovascular function, and physical performance.
How Is Lactate Measured?
A small blood sample is collected – typically from a fingertip, earlobe, or vein. The sample is then analyzed using a lactate analyzer or an enzymatic test system. Modern portable devices allow rapid on-site measurement, for example in a gym or on a sports field.
Normal Values and Reference Ranges
- Resting value: 0.5 – 2.0 mmol/L (millimoles per liter)
- Light physical activity: up to approx. 4.0 mmol/L
- Anaerobic threshold: approx. 4.0 mmol/L (varies individually)
- Critical value (lactic acidosis): above 5.0 mmol/L
When Is Lactate Measurement Used?

In clinical medicine, lactate measurement is used to detect life-threatening conditions at an early stage. An elevated lactate level may indicate the following diseases or conditions:
- Sepsis (severe bloodstream infection)
- Shock states (e.g., cardiogenic shock, septic shock)
- Myocardial infarction and severe heart failure
- Liver failure (since the liver is responsible for lactate clearance)
- Lactic acidosis – a dangerous drop in blood pH caused by lactate accumulation
- Poisoning, e.g., from carbon monoxide or certain medications (e.g., metformin)
Sports Medicine and Performance Diagnostics
In sports medicine, lactate measurement is a core tool for performance diagnostics. A lactate step test is used to determine the individual anaerobic threshold – the point at which the body produces more lactate than it can clear. Based on this data, training plans can be optimized and endurance performance systematically improved.
How the Lactate Step Test Works
During a step test, the workload is incrementally increased (e.g., on a treadmill or cycle ergometer). After each stage, a small blood sample is taken and the lactate value is measured. The resulting data is plotted as a lactate performance curve, which enables precise training control.
Clinical Significance of Elevated Lactate Levels
An elevated lactate level (hyperlactatemia) is an important indicator of inadequate tissue perfusion or increased anaerobic metabolism. In emergency and intensive care settings, lactate levels are monitored regularly, as they serve as a sensitive marker for the severity of shock and the prognosis of the patient.
Factors That Influence Lactate Levels
- Physical activity (acute exercise raises lactate)
- Nutritional status (fasting may affect lactate levels)
- Medications (e.g., metformin, epinephrine)
- Liver disease (reduced lactate clearance)
- Alcohol consumption (inhibits lactate metabolism)
