What Is a Bone Density Test?
A bone density test, also known as bone densitometry, is a diagnostic procedure used to measure the mineral density of bones. It provides important information about bone strength and helps identify the risk of fractures and osteoporosis (bone loss) at an early stage. The most widely used method is DXA (Dual-Energy X-ray Absorptiometry), which is considered the gold standard for measuring bone mineral density.
How Does a Bone Density Test Work?
In the DXA method, two X-ray beams with different energy levels are passed through the body. A detector measures how much radiation is absorbed by the bone tissue. The denser and more mineralized the bone, the more radiation is absorbed. Results are reported as a T-score (compared to a healthy young adult) and a Z-score (compared to people of the same age).
- T-score above -1.0: Normal bone density
- T-score between -1.0 and -2.5: Osteopenia (reduced bone density)
- T-score below -2.5: Osteoporosis
Who Should Have a Bone Density Test?
The test is particularly recommended for the following groups:
- Women aged 65 and older and men aged 70 and older
- Individuals with known risk factors such as family history of osteoporosis, low body weight, or smoking
- Postmenopausal women with additional risk factors
- People taking long-term corticosteroid medications
- Individuals who have experienced a fracture without significant trauma
What to Expect During the Test
A bone density test is painless and non-invasive. The patient lies on an examination table while a scanning arm moves slowly over the body. Measurements are typically taken at the lumbar spine and the femoral neck (hip), as these are the areas most commonly affected by osteoporosis-related fractures. The entire procedure usually takes between 10 and 20 minutes.
Understanding the Results
The test results help the physician assess the individual risk of fractures and determine whether treatment is needed. A low T-score indicates that bone density is below the normal range and that the risk of bone fractures is elevated. Combined with other risk factors, the doctor can decide whether medication, lifestyle changes, or further diagnostic measures are necessary.
Radiation Exposure and Safety
The radiation exposure from a DXA scan is very low – approximately one-tenth of that from a standard chest X-ray, and well below the level of daily background radiation. However, pregnant women should avoid the test or discuss it with their doctor beforehand.
