Bone marrow stimulation refers to medical interventions designed to increase the activity of the bone marrow and promote the production of blood cells. The bone marrow is the soft tissue found inside certain bones and is responsible for producing all types of blood cells: red blood cells (erythrocytes), white blood cells (leukocytes), and platelets (thrombocytes). When this system is impaired or requires targeted support, bone marrow stimulation procedures are used.
Indications
Bone marrow stimulation is applied in a variety of clinical situations:
- After chemotherapy or radiation therapy: Cancer treatments can damage the bone marrow and suppress blood cell production. Stimulation helps to accelerate recovery.
- Before stem cell donation: Donors receive growth factors to mobilize more stem cells into the bloodstream, where they can be collected.
- In bone marrow failure: Conditions such as aplastic anemia or myelodysplastic syndromes (MDS) impair blood cell formation.
- In severe neutropenia: A dangerous shortage of white blood cells that greatly increases the risk of infection.
- In orthopedic surgery: Techniques such as microfracture use bone marrow blood to support cartilage regeneration.
Methods of Bone Marrow Stimulation
Pharmacological Stimulation with Growth Factors
The most common approach involves administering hematopoietic growth factors (proteins that stimulate blood cell production). These include:
- G-CSF (Granulocyte Colony-Stimulating Factor): Agents such as filgrastim or lenograstim stimulate the production of white blood cells and are given as subcutaneous injections.
- GM-CSF (Granulocyte-Macrophage Colony-Stimulating Factor): Stimulates multiple cell lineages simultaneously.
- Erythropoietin (EPO): Promotes the formation of red blood cells and is used, for example, in chemotherapy-related anemia.
- Thrombopoietin receptor agonists: Increase platelet production; examples include romiplostim and eltrombopag.
Surgical Bone Marrow Stimulation
In orthopedic medicine, bone marrow stimulation describes a group of surgical techniques aimed at cartilage repair. By creating small, controlled injuries in the bone beneath the damaged cartilage, bone marrow blood rich in stem cells and growth factors is released. This forms a fibrin clot from which new cartilage tissue can develop. Established procedures include:
- Microfracture: Small holes are made in the bone using an awl or pick.
- Abrasion arthroplasty: The uppermost layer of bone is removed to release bone marrow blood.
- Subchondral drilling: Similar to microfracture but performed with a drill.
Mechanism of Action of Pharmacological Stimulation
The bone marrow contains hematopoietic stem cells -- precursor cells from which all blood cells originate. Growth factors bind to specific receptors on these cells and activate signaling pathways that promote cell division, maturation, and release into the bloodstream. G-CSF, for example, accelerates the maturation of neutrophil granulocytes and mobilizes stem cells from the bone marrow into the peripheral blood -- an effect that is specifically utilized for stem cell collection (apheresis).
Treatment Process
Treatment is individually planned based on the underlying condition and therapeutic goal:
- Regular blood counts (complete blood count, differential blood count) are performed to monitor progress.
- Growth factors are typically administered as daily injections over several days to weeks.
- In stem cell mobilization, apheresis (collection of stem cells from the blood) begins after several days of stimulation.
Side Effects and Risks
Pharmacological bone marrow stimulation is generally well tolerated but may cause side effects:
- Bone pain: Particularly in the spine, pelvis, and thigh bones -- a common but usually manageable symptom.
- Flu-like symptoms: Fatigue, fever, and headaches.
- Spleen enlargement (splenomegaly): In rare cases, the spleen may enlarge or, very rarely, rupture.
- Injection site reactions: Redness, swelling, or pain at the injection site.
- In very rare cases: increased risk of certain blood disorders with long-term use.
Bone Marrow Stimulation in Orthopedics
In orthopedic practice, bone marrow stimulation targets the regeneration of cartilage damage, especially in the case of focal cartilage defects in the knee joint. The resulting repair tissue is typically fibrocartilage, which is less durable than the original hyaline cartilage. Newer approaches such as the AMIC technique (autologous matrix-induced chondrogenesis) combine bone marrow stimulation with a collagen membrane to produce higher-quality tissue. Outcomes are best for smaller defects (under 2-4 cm²).
