What Is an End Artery?
An end artery (Latin: arteria terminalis) is an artery that supplies a specific tissue or organ as its sole arterial blood source. Unlike arteries that are connected to neighboring vessels via collaterals (bypass vessels), end arteries lack such connections. This means that if an end artery becomes blocked by a blood clot or narrowing, the tissue it supplies can no longer receive sufficient oxygen and begins to die -- a process known as an infarction.
Types of End Arteries
Anatomical End Artery
In a true anatomical end artery, there are absolutely no connections to neighboring vessels. Classic examples include the central artery of the retina (arteria centralis retinae) in the eye and the interlobular arteries of the kidney. Blockage of these vessels inevitably leads to the death of the supplied tissue.
Functional End Artery
A functional end artery may have anatomically present connections to neighboring vessels, but these connections are so small or so poorly developed that they cannot provide adequate alternative blood supply in the event of a vessel occlusion. Examples include the coronary arteries of the heart and certain arteries of the brain.
Clinical Significance
The concept of the end artery carries enormous clinical relevance, as occlusions of these vessels can lead to severe organ damage:
- Heart attack (myocardial infarction): Blockage of a coronary artery (functional end artery) leads to the death of heart muscle tissue.
- Stroke: Occlusion of cerebral arteries acting as functional end arteries causes death of brain tissue.
- Retinal artery occlusion: Blockage of the central artery of the retina can cause sudden, usually unilateral vision loss.
- Renal infarction: Blockage of renal arteries leads to the death of kidney tissue.
Diagnosis and Treatment
Since occlusions of end arteries represent medical emergencies, rapid diagnosis is essential. Key diagnostic methods include:
- Imaging procedures: MRI, CT angiography, or Doppler ultrasound to visualize the vessel occlusion.
- Laboratory tests: Measurement of biomarkers such as troponin in heart attacks or specific enzymes in organ damage.
Treatment depends on the affected organ and the extent of the damage. General therapeutic approaches include:
- Thrombolysis: Medication-based dissolution of the blood clot.
- Mechanical recanalization: Restoration of vessel patency through minimally invasive procedures (e.g., catheter techniques, stent implantation).
- Surgical intervention: In certain cases, surgical repair of the affected vessel.
Prevention
Since vessel occlusions are frequently caused by arteriosclerosis (hardening of the arteries), prevention of cardiovascular risk factors plays an important role:
- Control of high blood pressure, diabetes, and elevated blood lipid levels
- Smoking cessation
- Regular physical activity
- A healthy, balanced diet
