What is the Endorhachis?
The endorhachis (also spelled endorachis) is a delicate connective tissue membrane that lines the inner surface of the vertebral canal (canalis vertebralis). It forms an anatomical boundary layer between the bony and ligamentous structures of the spine and the epidural space, which contains adipose tissue, blood vessels, and nerve roots.
The term derives from the Greek words endon (within) and rhachis (spine), literally meaning the inner lining of the spinal column.
Anatomical Location and Structure
The endorhachis lies directly against the inner surface of the bony vertebral canal and is closely associated with the periosteum (bone membrane) of the vertebral bodies and arches, as well as the ligamentum flavum and other spinal ligaments. It forms the outer boundary of the epidural space (also called the peridural space), which lies between the endorhachis and the outer spinal meninx, the dura mater spinalis.

- Structure: Thin, fibrous connective tissue membrane
- Neighboring structures: Periosteum of the vertebrae, ligamentum flavum, epidural adipose tissue, epidural venous plexus
Functions of the Endorhachis
The endorhachis serves several important physiological roles:
- Protective function: It protects the epidural space and its contents from external mechanical forces.
- Delimitation of the epidural space: The endorhachis defines the outer border of the epidural space, thereby contributing to the controlled distribution of substances such as local anesthetics during epidural anesthesia.
- Structural integration: It connects the periosteum and ligaments of the spinal column, contributing to the mechanical stability of the vertebral canal.
Clinical Relevance
Although the endorhachis is rarely mentioned explicitly in everyday clinical language, it plays an important role in various medical procedures and conditions:
Epidural Anesthesia
During epidural anesthesia, a local anesthetic is injected precisely into the epidural space to block pain -- for example during surgery or labor. The correct identification of the epidural space, whose outer boundary is formed by the endorhachis, is essential for the success and safety of this procedure.
Epidural Hematomas and Abscesses
Under certain circumstances, epidural hematomas (collections of blood) or epidural abscesses (collections of pus) can develop within the epidural space. The endorhachis, as a delimiting structure, influences the spread of such pathological processes. Epidural hematomas may result from trauma, coagulation disorders, or as a complication of spinal procedures and often require emergency treatment.
Spinal Surgery
During surgical procedures on the spine, such as for disc herniation or spinal canal stenosis, surgeons must carefully identify and preserve anatomical layers including the endorhachis to avoid injury to the spinal cord and nerve roots.
Distinction from Related Structures
The endorhachis should not be confused with the three meningeal layers (meninges) that directly enclose the spinal cord:
- Dura mater spinalis: The outermost, tough meningeal layer surrounding the spinal cord
- Arachnoid mater spinalis: The middle, web-like meningeal layer
- Pia mater spinalis: The innermost, highly vascular layer directly covering the spinal cord
The endorhachis lies outside all three meningeal layers and lines the vertebral canal as the outermost inner layer. The epidural space is located between the endorhachis and the dura mater spinalis.
