The estradiol level refers to the measured concentration of estradiol (E2) in the blood. Estradiol is the most biologically active of the three naturally occurring estrogens in the human body. It is primarily produced in the ovaries, with smaller amounts made in the adrenal cortex and fatty tissue. In men, the testes and peripheral tissues produce small quantities of estradiol. Normal Values of the Estradiol Level
Reference ranges for the estradiol level vary depending on sex, age, and — in women — the phase of the menstrual cycle. Values are typically reported in picograms per milliliter (pg/mL) or picomoles per liter (pmol/L).
- Women in the follicular phase: 20–150 pg/mL
- Women around ovulation: 100–500 pg/mL
- Women in the luteal phase: 30–450 pg/mL
- Postmenopausal women: below 20 pg/mL
- Men: 10–40 pg/mL
- Prepubertal children: below 10 pg/mL

Causes of an Elevated Estradiol Level
An elevated estradiol level (hyperestrogenism) can result from various causes:

- Obesity, as adipose (fat) tissue produces estrogens

Diagnosis: Measuring the Estradiol Level
The estradiol level is determined through a simple blood draw. Blood is typically collected in the morning, and in women at a defined point in the menstrual cycle, since the value fluctuates significantly throughout the cycle. The physician evaluates the estradiol level alongside other hormone values — such as FSH (follicle-stimulating hormone), LH (luteinizing hormone), and progesterone — to obtain a comprehensive picture of the hormonal situation. Ultrasound examination of the ovaries or additional imaging techniques may also be used as supplementary diagnostics.
Treatment for Abnormal Estradiol Levels
Treatment depends on the underlying cause and the extent of the deviation:
- Hormone replacement therapy (HRT): In cases of estradiol deficiency, particularly during menopause, HRT with estradiol can be used to relieve symptoms and help prevent complications such as osteoporosis.
- Antiestrogens or aromatase inhibitors: When estradiol levels are too high — for example in breast cancer treatment — medications that suppress estrogen production or block its effects may be prescribed.
- Lifestyle modifications: Weight loss, a balanced diet, and moderate physical activity can positively influence hormone levels.
- Treatment of the underlying condition: If an organic cause is identified (e.g., an ovarian tumor), treating that condition takes priority.
