The RVO was considered one of the most far-reaching social laws of its time and served as the foundation of German social insurance law for more than six decades.

The RVO emerged from the tradition of Bismarckian social legislation of the late 19th century. Chancellor Otto von Bismarck had introduced the Health Insurance Act (1883), the Accident Insurance Act (1884), and the Old Age and Disability Insurance Act (1889), laying the groundwork for modern social insurance. The goal of the RVO was to consolidate these individual statutes, close legislative gaps, and create a clear, systematic legal framework for the protection of workers.
Scope and Structure
The Imperial Insurance Code was divided into several books, covering the following areas:
- Health Insurance: Compulsory insurance for workers and certain employees, providing benefits such as medical treatment, sickness benefits, and maternity benefits.
- Accident Insurance: Coverage for occupational accidents and diseases, funded entirely by employers.
- Invalidity and Survivors Insurance: Pension benefits in cases of reduced earning capacity, as well as provisions for widows and orphans.

Significance for Healthcare
In the area of healthcare, the RVO was of outstanding importance. It laid the legal foundation for what remains the core of the German statutory health insurance (gesetzliche Krankenversicherung, GKV) system to this day. For the first time, compulsory membership in a health fund (Krankenkasse), the definition of insured groups, and the catalogue of covered benefits were systematically codified. A key achievement was establishing a legal entitlement to medical care and sickness benefits for insured workers.
Evolution and Replacement
The RVO was amended and expanded numerous times over the decades but retained its core structure for much of the 20th century. It was gradually replaced by the Social Code (Sozialgesetzbuch, SGB), introduced from 1975 onward. The provisions governing healthcare were specifically superseded by SGB V (Statutory Health Insurance), which came into force in 1989. Other sections were replaced by SGB VI (pension insurance) and SGB VII (accident insurance).

Although the RVO is no longer in effect, its structural legacy continues to shape the German healthcare system today.
