What Is a Salicylic Acid Derivative?
A salicylic acid derivative is a chemical compound structurally derived from salicylic acid (2-hydroxybenzoic acid). Through various chemical modifications of the base structure, a wide range of compounds with different pharmacological properties can be produced. Salicylic acid derivatives are among the oldest and most widely used substances in the history of medicine.
Well-Known Representatives
- Acetylsalicylic acid (ASA): The most well-known salicylic acid derivative, marketed as Aspirin. It acts as an analgesic, antipyretic, anti-inflammatory, and antiplatelet agent.
- Salicylic acid: Used topically in dermatology for the treatment of warts, acne, psoriasis, and calluses.
- Salsalate: Another salicylic acid derivative with anti-inflammatory properties used in rheumatic conditions.
- Diflunisal: A nonsteroidal anti-inflammatory drug (NSAID) with analgesic and antipyretic effects.
- Mesalazine (5-aminosalicylic acid): Used in the treatment of inflammatory bowel diseases such as Crohn's disease and ulcerative colitis.
Mechanism of Action
Most salicylic acid derivatives act by inhibiting cyclooxygenase enzymes (COX-1 and COX-2). These enzymes are responsible for the synthesis of prostaglandins – signaling molecules that trigger pain, inflammation, and fever. By inhibiting these enzymes, inflammatory responses are reduced and pain is relieved.
Acetylsalicylic acid irreversibly inhibits COX-1, which permanently reduces platelet aggregation, making it valuable in cardiology for thrombosis prevention. Mesalazine, in contrast, acts primarily locally within the intestine and has a different therapeutic focus.
Medical Applications
Pain Relief and Fever Reduction
Salicylic acid derivatives such as acetylsalicylic acid are commonly used for mild to moderate pain, headaches, toothaches, and fever.
Cardiovascular Diseases
Low-dose acetylsalicylic acid (e.g., 100 mg daily) is used to prevent heart attacks and strokes in at-risk patients by inhibiting platelet aggregation.
Inflammatory Conditions
In rheumatoid arthritis, ankylosing spondylitis, and other inflammatory rheumatic diseases, salicylic acid derivatives can help alleviate symptoms.
Dermatology
Topically applied salicylic acid has a keratolytic effect – it dissolves hardened skin layers and is used for acne, psoriasis, warts, and corns.
Inflammatory Bowel Disease
Mesalazine is an important active ingredient in the treatment of ulcerative colitis and Crohn's disease and belongs to the group of aminosalicylate preparations.
Side Effects and Risks
Salicylic acid derivatives can cause adverse effects if used improperly:
- Gastric irritation and peptic ulcers: Especially with long-term use of acetylsalicylic acid
- Increased bleeding tendency: Due to the anticoagulant effect
- Allergic reactions: Salicylate intolerance can cause skin reactions, asthma attacks, or urticaria
- Reye syndrome: A rare but serious risk when acetylsalicylic acid is given to children under 12 with viral infections
- Kidney impairment: With overdose or long-term use
Contraindications and Drug Interactions
Salicylic acid derivatives should not be used in cases of:
- Known hypersensitivity to salicylates
- Active peptic ulcers or gastrointestinal bleeding
- Severe renal or hepatic impairment
- Concurrent use of anticoagulants without medical supervision
Interactions exist with blood thinners, other NSAIDs, methotrexate, and certain antidiabetic drugs.
