What Is Sebaceous Gland Inhibition?
Sebaceous gland inhibition describes measures or mechanisms that reduce the activity of the sebaceous glands (Glandulae sebaceae) in the skin. Sebaceous glands are small glands located in the dermis that produce sebum – a fatty secretion that protects and moisturises the skin. However, excessive sebum production, known as seborrhoea, can lead to skin problems such as acne, clogged pores, and oily skin. Targeted inhibition of sebaceous gland activity is therefore an important therapeutic approach in dermatology.
Causes of Excessive Sebum Production
Sebaceous glands are primarily stimulated by androgens (male sex hormones such as testosterone and DHT). Increased sebum production can be triggered by various factors:
- Hormonal changes: Puberty, menstrual cycle, pregnancy, or polycystic ovary syndrome (PCOS)
- Genetic predisposition: Family tendency towards oily skin or acne
- Stress: Elevated cortisol levels can stimulate sebum production
- Diet: High-glycaemic foods and dairy products are suspected to promote acne
- Medications: Certain corticosteroids or anabolic steroids can increase sebum production
Methods of Sebaceous Gland Inhibition
Pharmacological Treatment
The most effective pharmacological approach to sebaceous gland inhibition is isotretinoin (an oral retinoid), which can permanently reduce the size and activity of sebaceous glands. Further options include:
- Retinoids (topical or oral): Regulate cell turnover and reduce sebum production
- Antiandrogens: Substances such as spironolactone or certain hormonal contraceptives inhibit the androgenic stimulation of sebaceous glands
- Azelaic acid: A topical agent with sebum-regulating and antibacterial properties
Topical Active Ingredients
Various topically applied substances can locally influence sebum production:
- Niacinamide (Vitamin B3): Regulates sebum secretion and improves skin appearance
- Zinc: Inhibits the activity of the enzyme 5-alpha-reductase, which is involved in the androgenic stimulation of sebaceous glands
- Salicylic acid: Dissolves excess sebum and keratinised cells within the pores
- Green tea extract: Contains polyphenols with antiandrogenic properties
Physical and Aesthetic Procedures
Physical methods are also used in aesthetic dermatology to inhibit sebaceous gland activity:
- Laser treatments: Certain laser types (e.g. PDT – photodynamic therapy) can selectively damage sebaceous glands and reduce their activity long-term
- Chemical peels: Fruit acid or salicylic acid peels remove excess sebum and cleanse the pores
Clinical Relevance
Sebaceous gland inhibition plays a central role in the treatment of acne vulgaris, seborrhoeic dermatitis, and other seborrhoeic conditions. Effective reduction of sebum production prevents the accumulation of sebum in hair follicles, reduces the proliferation of the bacterium Cutibacterium acnes, and thereby decreases inflammation and blemish formation. The choice of appropriate method depends on the severity of the condition, the age of the patient, and any accompanying medical conditions.
