Tingling in the hands is a common symptom that may indicate nerve or circulatory disorders. Learn about causes, diagnosis, and treatment options.
What Is Tingling in the Hands?
Tingling in the hands – medically known as paresthesia – refers to an abnormal sensation such as tingling, numbness, burning, or a pins-and-needles feeling in the hands or fingers. This symptom can be temporary or chronic and is a frequent reason for medical consultation. Identifying the underlying cause is important, as tingling in the hands can indicate a wide range of conditions, from harmless to medically significant.
Causes
The causes of tingling in the hands are diverse. A distinction is made between short-term, harmless causes and chronic, medically relevant underlying conditions.
Common Harmless Causes
- Nerve pressure: Leaning on the arms for extended periods or sleeping in an awkward position can temporarily compress nerves and cause tingling.
- Hyperventilation: Rapid, shallow breathing during panic attacks can cause tingling in the hands and around the mouth.
- Cold exposure: Reduced blood flow due to cold temperatures can trigger temporary tingling sensations.
Medical Causes
- Carpal tunnel syndrome: Compression of the median nerve at the wrist, typically causing tingling in the thumb, index, and middle fingers.
- Cervical radiculopathy: Pinched nerves in the cervical spine that radiate tingling down the arm and into the hand.
- Polyneuropathy: Damage to the peripheral nerves, commonly associated with diabetes mellitus or chronic alcohol use.
- Raynaud syndrome: Vascular spasms causing temporary reduction of blood flow to the fingers.
- Multiple sclerosis: An inflammatory disease of the central nervous system that can cause tingling and numbness.
- Vitamin B12 deficiency: Low levels of vitamin B12 damage the myelin sheath of nerves, leading to tingling in the hands and feet.
- Thyroid disorders: Particularly an underactive thyroid (hypothyroidism) can be associated with tingling sensations.
- Stroke or TIA: Sudden one-sided tingling may be a sign of a stroke or transient ischemic attack (TIA) and requires immediate medical attention.
Symptoms and Associated Complaints
Tingling in the hands may occur alone or alongside other symptoms. Common associated complaints include:
- Numbness or weakness in the hand
- Pain or burning in the fingers
- Weakness when gripping objects
- Skin discoloration (white, blue, or red) in cold temperatures
- Tingling that worsens at night
If tingling appears suddenly, affects one side of the body, and is accompanied by speech difficulties, vision problems, or paralysis, emergency services should be contacted immediately, as this may indicate a medical emergency such as a stroke.
Diagnosis
Diagnosis begins with a thorough medical history and physical examination. The physician will ask about the location, duration, and triggering factors of the tingling. Depending on the suspected cause, the following investigations may be performed:
- Neurological examination: Assessment of reflexes, sensation, and muscle strength
- Blood tests: Blood glucose, vitamin B12, thyroid hormone levels, and inflammatory markers
- Electrophysiological tests: Nerve conduction studies (NCS) and electromyography (EMG) to assess nerve function
- Imaging: MRI of the cervical spine or brain if structural causes are suspected
- Ultrasound: Imaging of the carpal tunnel or blood vessels
Treatment
Treatment depends on the underlying cause of the tingling.
Conservative Measures
- Rest and relief of the affected area
- Physiotherapy and targeted nerve mobilization exercises
- Splints or braces (e.g., for carpal tunnel syndrome)
- Ergonomic adjustments in the workplace
Medication
- Pain-relieving medications (e.g., ibuprofen, diclofenac)
- Vitamin B12 supplementation when a deficiency is confirmed
- Corticosteroid injections for inflammatory causes
- Medications to improve circulation in Raynaud syndrome
Surgical Treatment
- Carpal tunnel release surgery
- Neurosurgical procedures for herniated discs causing nerve compression
When to See a Doctor
Temporary tingling after sleeping in an awkward position is usually harmless and resolves on its own. However, medical advice should be sought if the tingling:
- persists for more than a few days or recurs frequently,
- affects both hands or also the feet,
- is accompanied by muscle weakness, coordination problems, or other neurological symptoms,
- appears suddenly and is pronounced on one side of the body.
