What Is Blood Group Typing?
Blood group typing is a laboratory diagnostic procedure that identifies a person's blood group based on specific markers on the surface of red blood cells (erythrocytes). These markers are called antigens. The most widely used classification systems are the ABO system and the Rhesus (Rh) system.
Knowing one's blood type is essential in modern medicine, particularly for blood transfusions, surgical procedures, organ transplantation, and obstetric care.
The ABO System
The ABO system defines four blood types based on the presence or absence of specific antigens on red blood cells:
- Blood type A: Antigen A on red blood cells, Anti-B antibodies in the serum
- Blood type B: Antigen B on red blood cells, Anti-A antibodies in the serum
- Blood type AB: Both A and B antigens on red blood cells, no antibodies in the serum
- Blood type O: No A or B antigens on red blood cells, both Anti-A and Anti-B antibodies in the serum
Transfusing incompatible blood types can trigger a life-threatening haemolytic transfusion reaction, in which red blood cells are destroyed by the immune system.
The Rhesus System
After the ABO system, the Rhesus (Rh) system is the most clinically significant blood group system. The key marker is the Rh D antigen. Individuals who carry this antigen are classified as Rhesus-positive (Rh+), while those who do not are Rhesus-negative (Rh-).
The Rh factor is particularly important during pregnancy. If the mother is Rh-negative and the unborn child is Rh-positive, a condition known as Rh incompatibility can develop, potentially causing haemolytic disease of the newborn (HDN).
Other Blood Group Systems
Beyond ABO and Rh, several other blood group systems have clinical relevance, including:
- Kell system
- Kidd system
- Duffy system
- MNS system
These systems are especially important for patients who require regular blood transfusions, such as those with sickle cell disease or thalassaemia.
How Blood Group Typing Is Performed
Typing is carried out in a laboratory using a blood sample. Two main tests are typically performed:
- Forward grouping (cell typing): The patient's red blood cells are mixed with known test antisera to identify surface antigens.
- Reverse grouping (serum typing): The patient's serum is mixed with known test red blood cells to detect the presence of antibodies.
When both tests yield consistent results, the blood type is considered confirmed. Before any transfusion, a crossmatch (compatibility test) is also performed to verify that donor blood is compatible with the recipient's blood.
Clinical Significance
Blood group typing is indispensable in the following medical contexts:
- Blood transfusions: Prevents haemolytic reactions caused by incompatible blood
- Organ transplantation: Improves compatibility between donor organ and recipient
- Pregnancy and obstetrics: Early detection of Rh incompatibility and initiation of preventive treatment
- Forensic medicine: Personal identification
- Stem cell transplantation: Donor-recipient matching
