Structure and Function of the Mucosa
The mucosal lining of the GI tract consists of several protective layers. The innermost surface is covered by a layer of mucus (mucin), secreted by specialized goblet cells. This mucus layer protects the underlying epithelium from digestive acids and enzymes.
- Mucus layer: Buffers stomach acid and prevents direct contact between irritants and the epithelial surface.
- Epithelial cells: Form a tight cellular barrier and undergo regular renewal.
- Bicarbonate secretion: Neutralizes acid directly at the mucosal surface.
- Mucosal blood flow: Delivers oxygen and nutrients to support tissue repair and regeneration.
Causes of Impaired Mucosal Barrier Function
The protective function of the mucosal barrier can be compromised by various factors:
- Non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or diclofenac inhibit prostaglandin synthesis, which is essential for mucus production.
- Helicobacter pylori: This bacterium disrupts the gastric mucosal barrier and can cause gastritis and peptic ulcers.
- Chronic stress: Reduces mucosal blood flow and impairs mucus secretion.
- Alcohol and tobacco: Directly irritate the mucosa and impair its regenerative capacity.

Proton Pump Inhibitors (PPIs)

H2 Receptor Antagonists

Bismuth Compounds

- Reflux esophagitis: Inflammation of the esophageal lining caused by acid reflux
