What is a Wrist Ganglion?
A wrist ganglion (also called a ganglion cyst) is a benign, jelly-filled cyst that typically forms on the joint capsule or tendon sheaths of the wrist. It is the most frequently occurring benign soft tissue mass of the hand and wrist region. While generally harmless, a ganglion cyst can cause pain, tenderness, or limited range of motion depending on its size and location.
Causes
The exact cause of ganglion cysts is not fully understood. The following factors are commonly associated with their development:
- Joint or tendon irritation: Repetitive stress or minor injuries to the joint capsule or tendon sheath may trigger cyst formation.
- Degenerative changes: Wear and tear of the wrist joint may contribute to ganglion development.
- Trauma: Previous wrist injuries such as falls or sprains are considered possible triggers.
- Overuse: Occupations or sports that involve intensive wrist activity may increase the risk.
Symptoms
Common signs and symptoms of a wrist ganglion include:
- A visible, soft or firm lump on the back or palm side of the wrist
- Pain or tenderness, especially during movement or pressure
- Limited wrist mobility in cases of larger cysts
- Tingling or numbness if the cyst compresses a nearby nerve
- Fluctuating size of the lump, for example after physical activity
Diagnosis
Diagnosis is usually made clinically through visual inspection and palpation of the lump. Additional diagnostic tools may include:
- Transillumination: Shining a light through the cyst to confirm its fluid-filled nature.
- Ultrasound: Imaging to assess the cyst and its relationship to surrounding structures.
- MRI (Magnetic Resonance Imaging): Used for unclear cases or occult (non-visible) ganglions to precisely locate the cyst.
- X-ray: To rule out bony abnormalities or associated joint conditions.
Treatment
Many ganglion cysts resolve on their own and do not require active treatment. Available treatment options include:
Conservative Treatment
- Watchful waiting: For symptom-free ganglions, a wait-and-see approach is often recommended, as up to 50% of cysts resolve spontaneously.
- Immobilization: Resting and supporting the wrist can help reduce discomfort.
- Aspiration: The cyst is drained using a needle. This is a simple procedure, but recurrence rates are relatively high.
Surgical Treatment
- Surgical excision: Complete removal of the cyst including its stalk and connection to the joint capsule. This approach has the lowest recurrence rate.
- Arthroscopic removal: A minimally invasive technique using small incisions and a camera; offers faster recovery time.
Prognosis
Wrist ganglion cysts are benign and not life-threatening. After surgical removal, the recurrence rate is approximately 5–15%. Recurrence rates are higher following aspiration or conservative management. Regular follow-up is recommended after treatment.
