What is Genital Herpes?
Genital herpes is one of the most common sexually transmitted infections (STIs) worldwide. It is caused by the herpes simplex virus (HSV), most often by type 2 (HSV-2), and less frequently by type 1 (HSV-1), which more commonly causes oral cold sores. Once a person is infected, the virus remains in the body for life and can cause recurring outbreaks.
Causes and Transmission
Genital herpes is primarily transmitted through direct skin or mucous membrane contact during sexual activity. The virus can be passed on even when no visible sores are present, a process known as asymptomatic viral shedding.
- Transmission occurs through vaginal, anal, or oral sex
- Higher risk in individuals with weakened immune systems
- Transmission from mother to newborn during delivery is possible (neonatal herpes)
Symptoms
Many people with genital herpes experience no or only mild symptoms. When symptoms do occur, they typically include:
- Painful, itchy, or burning blisters in the genital, anal, or thigh area
- Open sores (ulcers) after the blisters burst
- Pain or burning sensation during urination
- Swollen lymph nodes in the groin
- General feeling of illness, fever, and headaches (especially during the first outbreak)
After the initial outbreak, recurrent episodes may occur, which are usually milder than the first infection.
Diagnosis
Diagnosis is typically made through physical examination and laboratory tests:
- Swab from blisters or sores: Detection of the virus using PCR (polymerase chain reaction) or viral culture
- Blood test (serology): Detection of antibodies against HSV-1 or HSV-2
Early diagnosis is important to prevent transmission and to start appropriate treatment.
Treatment
Genital herpes cannot be cured, as the virus remains permanently in the nervous system after infection. However, antiviral therapy can relieve symptoms, shorten outbreaks, and reduce the risk of transmission to sexual partners.
Antiviral Medications
- Acyclovir: A classic antiviral agent used to treat acute outbreaks
- Valacyclovir: A prodrug of acyclovir with better bioavailability, often used for suppressive therapy
- Famciclovir: Another antiviral option for both acute episodes and long-term management
Suppressive Therapy
For individuals with frequent recurrences (more than 6 per year) or those wishing to reduce transmission risk, daily antiviral suppressive therapy may be recommended.
Prevention
- Consistent use of condoms reduces -- but does not eliminate -- the risk of transmission
- Avoiding sexual contact during active outbreaks
- Open communication with sexual partners
- Regular STI screening for high-risk individuals

A genital herpes diagnosis can cause significant emotional distress. Shame, fear of stigma, and concerns about relationships are common reactions.
