What Is the Gleason Score?
The Gleason Score is an internationally recognized classification system used to assess the aggressiveness of prostate cancer (prostate carcinoma). It was developed in the 1960s by American pathologist Donald F. Gleason and remains one of the most important tools in prostate cancer diagnosis and treatment planning.
The score is based on the microscopic examination of prostate tissue samples obtained through a biopsy. A pathologist evaluates how much the cancer cells differ from normal prostate tissue — a measure known as the degree of differentiation. The more abnormal the cancer cells appear compared to healthy tissue, the more aggressive and fast-growing the tumor is considered to be.
How Is the Gleason Score Calculated?
When analyzing a biopsy, the pathologist identifies the two most common tissue growth patterns (graded 1 to 5) in the sample. These two grades are then added together to produce the Gleason Score:
- Primary Grade: The most predominant tumor growth pattern, scored 1–5.
- Secondary Grade: The second most common pattern, also scored 1–5.
- Gleason Score: The sum of both grades (e.g., 3 + 4 = 7).
The total score ranges from 2 to 10, although in modern clinical practice, scores below 6 are rarely assigned. Lower scores (6) indicate a less aggressive tumor, while higher scores (8–10) suggest a very aggressive cancer.
Gleason Grade Groups
Since the International Society of Urological Pathology (ISUP) updated the grading system in 2014, Gleason Scores are grouped into five Grade Groups to better reflect clinical significance:
- Grade Group 1: Gleason Score ≤ 6 – very low risk
- Grade Group 2: Gleason Score 3 + 4 = 7 – low to intermediate risk
- Grade Group 3: Gleason Score 4 + 3 = 7 – intermediate to high risk
- Grade Group 4: Gleason Score 8 – high risk
- Grade Group 5: Gleason Score 9–10 – very high risk
Clinical Significance and Application
The Gleason Score plays a crucial role in determining the most appropriate treatment for prostate cancer. In combination with the PSA (Prostate-Specific Antigen) level and the clinical stage of the tumor, it helps clinicians assess a patient´s individual risk profile and guide therapy decisions:
- Active Surveillance: Often recommended for low Gleason Score (6, Grade Group 1), avoiding immediate treatment.
- Surgery (Radical Prostatectomy): For localized tumors with intermediate to high Gleason Scores.
- Radiation Therapy: As an alternative or complement to surgery.
- Hormone Therapy and Chemotherapy: For advanced or metastatic prostate cancer with high Gleason Scores.
Limitations of the Gleason Score
Although the Gleason Score is a well-established tool, it has certain limitations. Because it is based on a biopsy that captures only part of the tumor tissue, it may occasionally underestimate or overestimate the true aggressiveness of the cancer. Modern imaging techniques such as multiparametric MRI (mpMRI) and molecular biomarker tests are increasingly used alongside the Gleason Score to improve diagnostic accuracy.
