What is a GLP-1 Receptor Agonist?
A GLP-1 receptor agonist is a class of medications that mimic the action of the naturally occurring hormone GLP-1 (glucagon-like peptide-1). GLP-1 is released by the intestines after a meal and plays a key role in regulating blood sugar levels and feelings of fullness. GLP-1 receptor agonists bind to the same receptor as the natural hormone and amplify its effects.
Indications
GLP-1 receptor agonists are primarily used for:
- Type 2 diabetes mellitus: to improve blood sugar control, either alone or in combination with other antidiabetic medications
- Obesity: to support weight loss in individuals with a body mass index (BMI) of 30 kg/m² or above, or 27 kg/m² or above with weight-related health conditions
- Cardiovascular risk reduction: in patients with type 2 diabetes and established cardiovascular disease
Mechanism of Action
GLP-1 receptor agonists work through several mechanisms:
- Stimulating insulin secretion: They prompt the pancreas to release insulin, but only when blood sugar levels are elevated, which means the risk of hypoglycemia (low blood sugar) is low.
- Suppressing glucagon: They inhibit the release of glucagon, a hormone that raises blood sugar levels.
- Slowing gastric emptying: Food moves more slowly from the stomach into the small intestine, prolonging the feeling of fullness.
- Reducing appetite: They act on hunger and satiety centers in the brain, leading to reduced food intake.
Known Active Substances
GLP-1 receptor agonists approved in Europe and the United States include:
- Semaglutide (e.g., Ozempic, Wegovy, Rybelsus)
- Liraglutide (e.g., Victoza, Saxenda)
- Dulaglutide (e.g., Trulicity)
- Exenatide (e.g., Byetta, Bydureon)
- Tirzepatide (e.g., Mounjaro) – a dual GIP/GLP-1 receptor agonist
Dosage and Administration
Depending on the active ingredient, GLP-1 receptor agonists are available as subcutaneous injections (under the skin, administered daily or weekly) or as an oral tablet (oral semaglutide, once daily). Doses are typically increased gradually to improve tolerability. The appropriate dose depends on the specific indication and the individual response of the patient.
Side Effects
GLP-1 receptor agonists are generally well tolerated but may cause the following side effects:
- Common: nausea, vomiting, diarrhea, constipation – especially at the start of treatment
- Occasional: abdominal pain, belching, reduced appetite
- Rare: pancreatitis (inflammation of the pancreas), gallbladder disease
- Very rare: thyroid tumors (observed mainly in animal studies; caution is advised in patients with a personal or family history of medullary thyroid carcinoma)
Hypoglycemia (low blood sugar) does not typically occur with GLP-1 receptor agonists alone, but may arise when combined with insulin or sulfonylureas.
Contraindications
GLP-1 receptor agonists should not be used in patients with:
- A personal or family history of medullary thyroid carcinoma
- Multiple endocrine neoplasia syndrome type 2 (MEN 2)
- Known hypersensitivity to the active substance
- Pregnancy or breastfeeding (insufficient safety data available)
