What is Gonadorelin?
Gonadorelin is a synthetically produced peptide hormone that is structurally identical to the naturally occurring Gonadotropin-Releasing Hormone (GnRH). In the body, GnRH is produced by the hypothalamus and controls the release of the gonadotropins Luteinizing Hormone (LH) and Follicle-Stimulating Hormone (FSH) from the pituitary gland. These hormones, in turn, regulate the function of the gonads (ovaries and testes), including the production of sex hormones and reproductive cells.
Mechanism of Action
Gonadorelin binds to specific GnRH receptors in the pituitary gland. When administered in a pulsatile manner – meaning in rhythmic, intermittent bursts at regular intervals – it mimics the natural secretion pattern of the hypothalamus and stimulates the release of LH and FSH. This leads to activation of the gonads and the production of sex hormones such as estrogen, progesterone, and testosterone.
In contrast, continuous administration paradoxically leads to desensitization of pituitary receptors and thus to suppression of gonadotropin release. This effect is exploited therapeutically, for example in hormone-dependent conditions.
Medical Applications
Diagnostic Use
In diagnostics, gonadorelin is used in the GnRH stimulation test to assess the integrity of the hypothalamic-pituitary-gonadal axis. The LH and FSH response following gonadorelin administration is measured to identify disorders of the pituitary gland or hypothalamus.
Treatment of Fertility Disorders
In women and men with hypothalamic amenorrhea or hypogonadotropic hypogonadism – a deficiency of GnRH from the hypothalamus – gonadorelin can be delivered in a pulsatile manner via a pump to restore the natural hormonal axis and promote fertility.
Delayed Puberty
In adolescents with significantly delayed puberty due to GnRH deficiency, gonadorelin may be used to initiate pubertal development.
Dosage Forms and Administration
Gonadorelin is available in various forms, including injection solutions (subcutaneous or intravenous) and nasal sprays. For pulsatile therapy, a portable micro-pump is commonly used, delivering the hormone at regular intervals of approximately 60 to 120 minutes. The exact dosage depends on the indication and the individual response of the patient, and is determined by the treating physician.
Side Effects
Gonadorelin is generally well tolerated. Possible side effects include:
- Local reactions at the injection site (redness, swelling)
- Headache
- Nausea
- Hot flushes
- In women: risk of ovarian hyperstimulation (particularly in fertility treatments)
- Rarely: allergic reactions
Contraindications and Interactions
Gonadorelin should not be used in individuals with known hypersensitivity to the active substance. Caution is advised in patients with hormone-dependent tumors. Interactions may occur with other hormones or hormonally active substances; therefore, medical consultation is essential before use.
