What is Gonadorelin Acetate?
Gonadorelin acetate is the acetate salt form of gonadorelin, a synthetically produced analogue of the naturally occurring gonadotropin-releasing hormone (GnRH). GnRH is produced in the hypothalamus and regulates the release of the gonadotropins LH (luteinizing hormone) and FSH (follicle-stimulating hormone) from the pituitary gland. These hormones in turn control gonadal function – including the production of sex hormones and the maturation of eggs and sperm.
Mechanism of Action
Gonadorelin acetate binds to specific GnRH receptors in the pituitary gland. When administered in a pulsatile manner – that is, in regular, brief pulses – it mimics the physiological secretion of endogenous GnRH. This stimulates the pituitary to release LH and FSH, which in turn activate the gonads.
Unlike long-acting GnRH agonists, which cause desensitisation of the pituitary and suppression of hormone production with continuous administration, gonadorelin acetate achieves a physiological stimulation of the hormonal axis when used in a pulsatile fashion.
Indications
Diagnostic Use
In diagnostics, gonadorelin acetate is used as a GnRH stimulation test to evaluate the function of the hypothalamic-pituitary-gonadal axis. The pituitary response to a GnRH stimulus is measured, providing information about possible hypofunction.
Treatment of Hypogonadism
Gonadorelin acetate is used to treat hypothalamic hypogonadism – a condition in which the hormonal axis is disrupted due to a defect in the hypothalamus. Pulsatile delivery via a specialised pump can replace the natural GnRH signal.
Fertility Treatment
In both women and men with hypothalamically caused infertility, gonadorelin acetate can stimulate the maturation of eggs or sperm, thereby restoring fertility. In women, it can induce ovulation.
Dosage and Administration
Gonadorelin acetate is typically administered intravenously or subcutaneously via a pulsatile infusion pump. The pulse dose and frequency are individually adjusted to replicate physiological GnRH secretion (typically every 60–120 minutes). The exact dosage is always determined and regularly monitored by the treating physician.
Side Effects
Gonadorelin acetate is generally well tolerated. Possible side effects include:
- Local reactions at the injection site (redness, swelling, pain)
- Headache
- Nausea
- In women: risk of ovarian hyperstimulation syndrome
- Rarely: allergic reactions
- With prolonged use: risk of infection at the infusion site
Contraindications
Gonadorelin acetate should not be used in cases of:
- Known hypersensitivity to gonadorelin or any of its excipients
- Hormone-sensitive tumours (e.g., hormone-dependent carcinomas)
- Pregnancy (unless specifically indicated by a physician)
- Conditions in which pregnancy is not desired
