What is L-Thyroxine?
L-Thyroxine (active ingredient: levothyroxine) is a synthetically produced thyroid hormone that is chemically identical to the naturally occurring human hormone thyroxine (T4). It is one of the most commonly prescribed medications worldwide and is primarily used to treat hypothyroidism (underactive thyroid gland).
Indications – When is L-Thyroxine Used?
L-Thyroxine is prescribed in a variety of medical situations:
- Hypothyroidism: Replacement of insufficient thyroid hormone production, for example in Hashimoto thyroiditis or following surgical removal of the thyroid gland.
- Goiter (enlarged thyroid): Reduction of thyroid gland size by suppressing thyroid-stimulating hormone (TSH).
- Thyroid cancer: Hormone replacement after total thyroidectomy and TSH-suppression therapy to reduce the risk of recurrence.
- Congenital hypothyroidism: Early treatment in newborns to prevent developmental disorders.
Mechanism of Action
L-Thyroxine acts as a synthetic analogue of the thyroid hormone T4. After oral ingestion, it is absorbed in the intestine and enters the bloodstream. Inside body cells, T4 is converted by the enzyme deiodinase into the biologically more active form triiodothyronine (T3). T3 then binds to specific receptors in the cell nucleus and regulates the activity of numerous genes involved in metabolism, growth, cardiac function, and the nervous system.
By supplementing L-Thyroxine, TSH levels produced by the pituitary gland are normalized or intentionally suppressed, thereby relieving the thyroid gland and inhibiting its pathological growth.
Dosage and Administration
L-Thyroxine is typically taken orally as a tablet or oral solution. The following points are important for correct administration:
- Take in the morning on an empty stomach, at least 30 minutes before breakfast, with a full glass of water.
- Dosage is adjusted individually based on blood test results (TSH, fT3, fT4).
- Typical maintenance doses for adults range between 25 and 200 micrograms per day.
- Certain foods (e.g., coffee, soy products, calcium) and medications can impair absorption and should be taken at a different time.
Side Effects
When correctly dosed, L-Thyroxine is generally well tolerated. However, overdosage can cause symptoms of hyperthyroidism (overactive thyroid):
- Rapid heartbeat (tachycardia), cardiac arrhythmias
- Restlessness, nervousness, sleep disturbances
- Weight loss, excessive sweating
- Tremor, headaches
- Heat intolerance, diarrhea
If any of these symptoms occur, a physician should be consulted promptly to reassess the dosage.
Special Warnings and Contraindications
L-Thyroxine should not be used, or only used under strict medical supervision, in patients with:
- Untreated heart conditions (e.g., coronary artery disease, heart failure)
- Untreated adrenal insufficiency
- Acute myocardial infarction
During pregnancy, adequate thyroid hormone levels are especially important for the development of the child. The required dose of L-Thyroxine may increase during pregnancy, making regular monitoring essential.
