A lateral ankle ligament tear is a complete rupture of the outer ligaments of the ankle joint. It is most often caused by rolling the ankle inward and is one of the most common sports injuries.
What Is a Lateral Ankle Ligament Tear?
A lateral ankle ligament tear (also called a lateral ligament rupture) is the complete tearing of one or more ligaments on the outer side of the ankle joint. The most commonly affected structures are the anterior talofibular ligament (ATFL) and the calcaneofibular ligament (CFL). This injury is among the most frequent musculoskeletal injuries in both sports and everyday life.
Causes
The classic mechanism of injury is a sudden inward rolling of the foot (supination trauma), which overstretches and tears the outer ligaments. Common triggers include:
- Sports involving rapid direction changes (football, basketball, tennis)
- Stumbling on uneven ground
- Missteps on stairs
- Insufficient warm-up or pre-existing ankle instability
Symptoms
Following the injury, patients typically experience:
- Sharp pain on the outer side of the ankle
- Swelling and bruising (haematoma) around the lateral malleolus
- Tenderness on palpation over the ligament area
- A sense of instability when bearing weight
- Restricted range of motion in the ankle joint
Diagnosis
Diagnosis is based on a combination of clinical examination and imaging:
- Physical examination: The physician assesses joint stability, pain, and mobility. Key tests include the anterior drawer test and the talar tilt test.
- X-ray: To rule out fractures of the ankle or foot bones
- Ultrasound: To visualise the ligaments and assess the extent of the rupture
- MRI (Magnetic Resonance Imaging): Used in complex cases for precise evaluation of ligament damage and surrounding structures
Treatment
Conservative Treatment
The majority of lateral ankle ligament tears are managed without surgery. Immediately after the injury, the RICE protocol is recommended:
- Rest – stop all activity immediately
- Ice – apply ice to reduce swelling (never directly on skin)
- Compression – apply a compression bandage
- Elevation – elevate the limb to minimise swelling
Further treatment typically includes:
- Supportive ankle braces or orthoses worn for several weeks
- Physiotherapy to strengthen the surrounding muscles and restore joint stability
- Pain relief medication such as ibuprofen or diclofenac as needed
Surgical Treatment
Surgery is rarely necessary and is usually considered in cases of:
- Chronic ankle instability following repeated ligament injuries
- Failure of conservative treatment
- Associated injuries such as cartilage damage
Common surgical procedures include ligament repair (direct suture) or ligament reconstruction using autologous tissue grafts.
Recovery and Prognosis
With appropriate treatment, most lateral ankle ligament tears heal well. Mild injuries typically recover within 4 to 6 weeks, while severe cases may require 3 to 6 months. A structured rehabilitation programme is essential to prevent re-injury and restore full function.
