Lochia is the normal vaginal discharge that occurs after childbirth. It consists of blood, mucus, and uterine tissue shed as the uterus returns to its pre-pregnancy size and condition. Phases and Progression of Lochia
Lochia progresses through three distinct stages, each characterized by changes in color, consistency, and volume:
- Lochia rubra (Days 1–4): The discharge is bright to dark red and consists mainly of blood, decidual tissue, and mucus. It may resemble a heavy menstrual period.
- Lochia serosa (Days 5–10): The discharge becomes lighter in color, turning pink to brownish. It contains a mixture of blood, wound secretions, and white blood cells.
- Lochia alba (Day 10 to Week 6): The final phase features a yellowish-white discharge composed mainly of leukocytes, mucus, and epithelial cells. It gradually decreases until it stops completely.
The total duration varies between individuals but typically resolves within 4 to 6 weeks after birth.
Normal Appearance vs. Warning Signs
While lochia is a normal physiological process, certain signs should prompt immediate medical attention:
- Sudden increase in bleeding or passage of large blood clots
- Foul-smelling or offensive odor, which may indicate infection
- Fever above 38 degrees Celsius (100.4 degrees Fahrenheit)
- Severe abdominal pain or uterine tenderness
- Discharge that restarts or turns bright red after it had appeared to stop
Possible Complications
Subinvolution of the Uterus
Subinvolution occurs when the uterus fails to contract and return to its normal size at the expected rate. This can cause prolonged or recurrent bleeding and may be associated with retained placental fragments or infection.
Endometritis
Endometritis is an infection of the inner lining of the uterus. Symptoms include foul-smelling lochia, fever, and lower abdominal pain. It is treated with antibiotics.
Postpartum Hemorrhage
Excessive bleeding after delivery is known as postpartum hemorrhage and is a medical emergency requiring immediate treatment.
Hygiene and Practical Recommendations
Proper hygiene during the lochia period is important to reduce the risk of infection:
- Use only maternity pads or sanitary towels – tampons should be avoided as they increase the risk of infection
- Change pads regularly, approximately every 2 to 4 hours
- Clean the genital area gently with warm water
- Avoid baths, swimming pools, and saunas during the early postpartum period
- Refrain from sexual intercourse until the lochia has fully stopped
The Role of Breastfeeding
Breastfeeding stimulates the release of oxytocin, a hormone that promotes uterine contractions and speeds up the return of the uterus to its normal size. Breastfeeding mothers may notice a temporary increase in lochia during or after nursing – this is a normal and positive sign of uterine recovery.
When Does Lochia End?
In most women, lochia resolves within 4 to 6 weeks postpartum. Women who are not breastfeeding may experience a return of their menstrual cycle shortly after lochia ends. Breastfeeding mothers often experience a delayed return of menstruation due to elevated prolactin levels suppressing ovulation.
