What is Tirzepatide?
Tirzepatide is an innovative active substance belonging to the class of dual incretin receptor agonists. It is a synthetic peptide molecule that simultaneously binds to two different hormone receptors: the GIP receptor (glucose-dependent insulinotropic polypeptide) and the GLP-1 receptor (glucagon-like peptide-1). Tirzepatide is marketed under the brand name Mounjaro and is approved for the treatment of type 2 diabetes mellitus and for weight management in adults with obesity.
Mechanism of Action
Tirzepatide mimics the effects of two naturally occurring gut hormones released after food intake. By simultaneously activating GIP and GLP-1 receptors, the drug exerts several complementary effects:
- Stimulation of insulin secretion: Tirzepatide stimulates the pancreas to release more insulin when blood glucose levels are elevated, thereby lowering blood sugar.
- Inhibition of glucagon release: The drug reduces the secretion of glucagon, a hormone that raises blood sugar levels.
- Delayed gastric emptying: Digestion is slowed down, which blunts the rise in blood glucose after meals.
- Appetite reduction: Through central mechanisms in the brain, the feeling of fullness is enhanced and appetite is suppressed, leading to reduced caloric intake.
- Improved insulin sensitivity: Tirzepatide enhances the responsiveness of body cells to insulin.
Indications
Tirzepatide is approved for the following uses:
- Type 2 diabetes mellitus: To improve blood sugar control in adults, in combination with diet and physical activity, as monotherapy or combination therapy.
- Obesity and overweight: For weight management in adults with a body mass index (BMI) of 30 kg/m² or more, or from a BMI of 27 kg/m² in the presence of weight-related comorbidities.
Dosage and Administration
Tirzepatide is administered once weekly as a subcutaneous injection (under the skin), typically into the abdomen, thigh, or upper arm. Treatment is initiated at a low dose and gradually increased to improve tolerability. Standard doses range from 2.5 mg to 15 mg per week, depending on the therapeutic goal and individual tolerability.
Side Effects
Like all medications, tirzepatide can cause side effects. The most common are gastrointestinal in nature and occur mainly at the start of therapy or after a dose increase:
- Nausea
- Vomiting
- Diarrhea
- Constipation
- Abdominal pain
Rarer but more serious side effects may include:
- Pancreatitis (inflammation of the pancreas)
- Cholecystitis (gallbladder inflammation) or gallstones
- Increased heart rate
- Injection site reactions
Tirzepatide is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 (MEN 2).
Clinical Evidence
In the clinical trial programs SURPASS (for type 2 diabetes) and SURMOUNT (for obesity), tirzepatide demonstrated impressive results. In patients with type 2 diabetes, HbA1c values (long-term blood glucose marker) were significantly reduced. In the obesity trials, participants receiving the highest dose (15 mg) achieved an average body weight reduction of up to 22.5%, substantially exceeding the efficacy of comparable therapies.
