What is Atorvastatin?
Atorvastatin is a cholesterol-lowering medication belonging to the class of drugs known as statins (HMG-CoA reductase inhibitors). It is available by prescription and is used to treat elevated blood lipid levels and to reduce the risk of heart attack, stroke, and other cardiovascular events. Atorvastatin is one of the most widely prescribed medications in the world.
Indications
Atorvastatin is indicated for:
- Hypercholesterolaemia: elevated LDL cholesterol levels in the blood
- Mixed dyslipidaemia: simultaneously elevated cholesterol and triglyceride levels
- Primary prevention: reducing the risk of heart attack and stroke in high-risk patients
- Secondary prevention: following a heart attack, stroke, or in patients with coronary artery disease
- Familial hypercholesterolaemia: genetically caused severely elevated cholesterol levels
Mechanism of Action
Atorvastatin inhibits the enzyme HMG-CoA reductase, which catalyses a key step in cholesterol biosynthesis in the liver. By blocking this enzyme, the liver produces less cholesterol. In response, the liver upregulates the number of LDL receptors on its surface, which then remove LDL cholesterol from the bloodstream. This leads to a significant reduction in circulating LDL cholesterol (often referred to as bad cholesterol).
Dosage
The usual starting dose is 10 to 20 mg once daily. Depending on the therapeutic goal and individual tolerability, the dose may be increased up to 80 mg per day. Atorvastatin can be taken with or without food and should ideally be taken at the same time each day. The appropriate dose is determined by a physician.
Side Effects
Common Side Effects
- Muscle pain or weakness (myalgia)
- Headache
- Nausea and gastrointestinal discomfort
- Elevated liver enzymes (transaminases)
Rare but Serious Side Effects
- Rhabdomyolysis: severe muscle breakdown that can lead to kidney failure (very rare)
- Myopathy: muscle disease characterised by severe pain and loss of strength
- Increased risk of type 2 diabetes with long-term use
- Liver dysfunction
Patients should seek immediate medical advice if they experience unusual muscle pain, dark-coloured urine, or severe weakness.
Drug Interactions
Atorvastatin is metabolised by the liver enzyme CYP3A4. The following substances may affect its efficacy or side effect profile:
- Grapefruit juice: inhibits CYP3A4 and can raise atorvastatin plasma levels
- Ciclosporin, clarithromycin, itraconazole: increase the risk of muscle damage
- Fibrates (e.g. gemfibrozil): concurrent use increases the risk of myopathy
- Anticoagulants (e.g. warfarin): the anticoagulant effect may be enhanced
Contraindications
- Active liver disease or persistently elevated liver enzymes
- Pregnancy and breastfeeding (teratogenic)
- Known hypersensitivity to atorvastatin
Treatment Context
Atorvastatin is typically used as part of a comprehensive treatment plan that also includes lifestyle modifications. These include a low-cholesterol diet, regular physical activity, smoking cessation, and weight management. In most cases, treatment is long-term, as the cholesterol-lowering effect diminishes after the medication is discontinued.
