What Is an Aneurysm?
An aneurysm is an abnormal, localized bulge or ballooning in the wall of an artery caused by weakness in the vessel wall. As blood pressure pushes against the weakened area, the artery expands and can eventually rupture. Aneurysms can develop in any artery throughout the body, but they most commonly occur in the aorta (the main artery of the body), in the arteries of the brain (intracranial aneurysms), and in the arteries of the abdomen and legs.
Causes of an Aneurysm
Several factors can weaken artery walls and lead to aneurysm formation:
- Atherosclerosis: Build-up of plaques that weakens and hardens artery walls.
- High blood pressure (hypertension): Persistently elevated pressure stresses the vessel walls.
- Genetic conditions: Connective tissue disorders such as Marfan syndrome or Ehlers-Danlos syndrome increase risk.
- Smoking: Significantly damages arterial walls over time.
- Inflammation of the vessel wall due to infections or autoimmune diseases.
- Trauma or injury that damages the arterial wall.
Symptoms of an Aneurysm
Many aneurysms are asymptomatic and are discovered incidentally during imaging studies performed for other reasons. However, depending on the location and size of the aneurysm, warning signs can occur.
Symptoms of an Abdominal Aortic Aneurysm
- A persistent, dull ache in the abdomen or lower back
- A pulsating sensation in the abdomen
- Feeling of pressure or fullness in the abdomen
- If ruptured: sudden, severe abdominal or back pain, circulatory shock, loss of consciousness – medical emergency
Symptoms of a Brain (Intracranial) Aneurysm
- Sudden, severe headache often described as the worst headache of one's life (thunderclap headache)
- Vision disturbances, double vision, or blurred vision
- Numbness or weakness in the face or limbs
- Speech difficulties
- Sensitivity to light, stiff neck
- If ruptured: sudden loss of consciousness, extreme headache – life-threatening emergency
Symptoms of Aneurysms in Other Vessels
- Swelling, pulsating sensation, or pain in the affected body region (e.g., behind the knee, in the groin)
- Tingling or numbness caused by pressure on nearby nerves
- Reduced blood flow to the limbs
Diagnosis
Because aneurysms frequently cause no or only vague symptoms, targeted diagnostic testing is essential:
- Ultrasound (sonography): A simple, radiation-free method widely used to detect abdominal aortic aneurysms.
- Computed Tomography (CT scan): Provides detailed images of blood vessels and the extent of the aneurysm.
- Magnetic Resonance Imaging (MRI): Particularly useful for imaging brain aneurysms.
- Angiography: An X-ray technique using contrast dye to visualize blood vessels in detail.
Treatment
Treatment depends on the location, size, and growth rate of the aneurysm, as well as the overall health of the patient:
- Conservative management: Regular monitoring, blood pressure control, and smoking cessation for small, stable aneurysms.
- Open surgical repair: The affected section of the vessel is surgically removed and replaced with a graft.
- Endovascular repair: A minimally invasive procedure where a stent-graft is inserted through the groin arteries to reinforce the weakened vessel.
- Coiling or clipping: Specialized neurosurgical techniques used to treat brain aneurysms by cutting off blood flow to the aneurysm.
When to Seek Medical Attention
If you experience a sudden, extremely severe headache, chest pain, or abdominal or back pain of unknown origin that worsens rapidly, call emergency services immediately. A ruptured aneurysm is a life-threatening emergency requiring immediate intervention.
