What Is the Glucose Metabolism Test?
The glucose metabolism test is a diagnostic procedure that assesses how efficiently the body absorbs, processes, and regulates glucose (blood sugar). Glucose is the primary energy source for the human body, and a disrupted glucose metabolism can indicate conditions such as type 2 diabetes, type 1 diabetes, insulin resistance, or gestational diabetes. The test provides important information about pancreatic function and the effectiveness of the hormone insulin.
How the Test Works
During a glucose metabolism test, the patient is given a defined amount of glucose – typically in the form of a drinking solution. Blood samples are then taken at specific time intervals to measure blood sugar levels. This allows clinicians to observe how quickly and completely the body breaks down the ingested glucose.
Fasting Blood Glucose
The test often begins with a fasting blood glucose measurement. The patient must refrain from eating or drinking (except water) for several hours prior to the test. An elevated fasting blood glucose level can be an early indicator of a metabolic disorder.
Oral Glucose Tolerance Test (oGTT)
The most common form of the glucose metabolism test is the oral glucose tolerance test (oGTT). The patient drinks a solution containing 75 grams of glucose. Blood measurements are taken while fasting and then one and two hours after ingestion. The results are used to determine whether glucose tolerance is normal, impaired, or consistent with a diagnosis of diabetes.
When Is the Test Performed?
The glucose metabolism test is used in a variety of clinical situations:
- Suspected type 2 diabetes or prediabetes
- Screening in high-risk patients (e.g., obesity, family history, high blood pressure)
- Diagnosis and monitoring of gestational diabetes during pregnancy
- Evaluation of symptoms such as frequent urination, excessive thirst, or unexplained weight loss
- Monitoring of known metabolic conditions
Interpretation of Results
The results of the glucose metabolism test are evaluated against internationally recognized threshold values. For the oral glucose tolerance test (75 g), the following reference values apply:
- Fasting value: Below 100 mg/dL is considered normal; 100–125 mg/dL suggests impaired fasting glucose; 126 mg/dL or above raises suspicion of diabetes.
- 2-hour value: Below 140 mg/dL is normal; 140–199 mg/dL indicates impaired glucose tolerance; 200 mg/dL or above is consistent with diabetes mellitus.
These values serve as a general guide. A definitive diagnosis is always made by a physician taking into account additional clinical findings.
Preparation and Procedure
To ensure reliable test results, the following points should be observed:
- At least 8 hours of fasting before the test (no food, no sweetened beverages)
- A carbohydrate-rich diet in the three days prior to the test (at least 150 g of carbohydrates per day)
- Physical rest during the test
- No medications that affect blood sugar should be taken without consulting a doctor first
- No smoking during the test
Risks and Side Effects
The glucose metabolism test is a safe procedure with very low risks. Possible side effects of the glucose solution include mild nausea or dizziness. In individuals with known diabetes mellitus, a significant rise in blood sugar may occur; in such cases, the test is performed under medical supervision.
