What Is Angiogenesis Inhibition?
Angiogenesis inhibition (also referred to as anti-angiogenesis or antiangiogenic therapy) is the deliberate blocking of angiogenesis – the process by which new blood vessels form from pre-existing ones. Under normal conditions, angiogenesis plays a vital role in wound healing and tissue growth. However, in pathological states such as cancer and certain eye diseases, uncontrolled blood vessel growth can be harmful. Angiogenesis inhibitors are drugs or substances that specifically target and block this process.
The Role of Angiogenesis in Disease
Beyond a certain size, tumors require their own blood supply to receive oxygen and nutrients. To achieve this, they secrete signaling proteins – most notably Vascular Endothelial Growth Factor (VEGF) – that stimulate the growth of new blood vessels. Without this vascular network, tumors cannot continue to grow or spread to other parts of the body (metastasis). Angiogenesis inhibition aims to block this critical step in tumor progression.
Mechanism of Action
Angiogenesis inhibitors work through several mechanisms:
- VEGF antibodies (e.g., bevacizumab): Bind directly to VEGF, preventing it from activating its receptor on blood vessel cells.
- VEGF receptor inhibitors (e.g., sorafenib, sunitinib): Block the intracellular signaling cascade within endothelial cells, stopping vessel growth at the cellular level.
- Endogenous inhibitors (e.g., angiostatin, endostatin): Naturally occurring proteins that regulate vascular growth and can be used therapeutically.
Areas of Application
Oncology (Cancer Therapy)
The most important application of angiogenesis inhibition is in cancer treatment. Angiogenesis inhibitors are often used in combination with chemotherapy or other targeted therapies. Approved indications include:
- Colorectal cancer
- Non-small cell lung cancer
- Renal cell carcinoma
- Ovarian cancer
- Hepatocellular carcinoma (liver cancer)
Ophthalmology
In eye care, anti-VEGF agents (e.g., ranibizumab, aflibercept) are used to treat wet age-related macular degeneration (AMD) and diabetic macular edema. By inhibiting pathological blood vessel growth in the retina, these drugs can slow or halt vision loss.
Side Effects
Because angiogenesis inhibitors also affect normal physiological vascular processes, a range of side effects can occur:
- High blood pressure (hypertension)
- Increased risk of bleeding
- Impaired wound healing
- Thromboembolic events (blood clots)
- Proteinuria (protein in the urine)
- Gastrointestinal perforation (rare but serious)
Current Research and Outlook
Research into angiogenesis inhibition remains an active and rapidly evolving field. Combination therapies pairing angiogenesis inhibitors with immune checkpoint inhibitors are showing promising results across multiple cancer types. Scientists are also investigating how to overcome resistance mechanisms that tumors develop against anti-angiogenic drugs, with the goal of improving long-term treatment outcomes.
