What is Annuloplasty?
Annuloplasty is a cardiac surgical technique in which the valve ring (annulus) of a heart valve is reconstructed or stabilized. It is commonly performed as part of heart valve repair surgery to correct a dilated or deformed valve opening, thereby restoring normal valve function. The procedure is most frequently applied to the mitral valve but can also be used for the tricuspid valve.
Indications for Annuloplasty
Annuloplasty is indicated when the valve annulus has become so enlarged or distorted that the valve leaflets can no longer close properly. Common indications include:
- Mitral valve regurgitation: incomplete closure of the mitral valve allowing blood to flow backwards
- Tricuspid regurgitation: functional dilation of the tricuspid valve annulus
- Degenerative valve disease: e.g., due to connective tissue weakness or aging
- Inflammatory heart conditions: e.g., following rheumatic fever
- Functional valve insufficiency resulting from heart muscle disease (cardiomyopathy)
How Does Annuloplasty Work?
The goal of the procedure is to reduce the dilated valve annulus to its correct size and permanently stabilize it. Two principal techniques are used:
Ring Annuloplasty
In ring annuloplasty, an artificial ring -- either rigid, semi-rigid, or flexible, made from metal and/or synthetic material -- is sewn around the valve annulus. This ring restores the correct shape and prevents re-dilation. Depending on the clinical situation, complete (full) or partial (semi-circular) rings are used.
Suture Annuloplasty
Suture annuloplasty (also known as the De Vega technique) reduces the valve annulus through targeted suturing without implanting a foreign body. This method is frequently used for the tricuspid valve.
Surgical Approach and Procedure
Annuloplasty is typically performed under general anesthesia using a cardiopulmonary bypass machine, which takes over the pumping and oxygenation functions of the heart during the operation. Access is classically achieved via a sternotomy (incision through the breastbone), but minimally invasive approaches (small incisions between the ribs) and catheter-based techniques are increasingly used, particularly in high-risk patients.
Benefits and Outcomes
Annuloplasty is a well-established procedure with favorable long-term outcomes. In many cases it allows preservation of the natural heart valve, which offers several advantages over complete valve replacement:
- No lifelong anticoagulation therapy required (with biological or native valves)
- Better cardiac function through preservation of natural valve mechanics
- Lower risk of infection associated with prosthetic valves
- Improved quality of life and physical capacity
Potential Risks and Complications
As with all cardiac surgical procedures, annuloplasty carries certain risks:
- Bleeding or postoperative hemorrhage
- Infections (including endocarditis)
- Cardiac arrhythmias
- Stroke or other thromboembolic events
- Failure of the repair with recurrence of valve regurgitation
- Rare reactions to the ring material
Individual risk depends on the patient's age, general health, and comorbidities, and is carefully evaluated before the procedure.
Aftercare and Rehabilitation
Following annuloplasty, patients are initially monitored in the intensive care unit, followed by a hospital stay of typically 7 to 14 days. Cardiac rehabilitation is recommended afterward to support recovery of heart function. Regular echocardiography follow-up (heart ultrasound) is necessary to assess long-term surgical success.
