What Does Acetabular Mean?
Acetabular is a medical adjective used to describe structures, processes, or conditions that involve the acetabulum – the socket of the hip joint. The acetabulum is a cup-shaped cavity in the pelvis that receives the head of the femur (thighbone), forming the hip joint. The term derives from the Latin word acetabulum, meaning a small vinegar cup, referring to the shape of the socket.
Anatomy of the Acetabulum
The acetabulum is formed by the fusion of three pelvic bones that remain separate during childhood and unite in adulthood:
- Ilium – forming the upper portion
- Ischium – forming the posterior and inferior portion
- Pubis – forming the anterior portion
The articular surface of the acetabulum is crescent-shaped (lunate surface) and lined with cartilage. The rim of the socket is deepened and reinforced by a ring of fibrocartilage called the acetabular labrum, which improves joint stability and helps keep the femoral head in place.

Acetabular Fractures

Acetabular Dysplasia
Hip dysplasia (dysplasia coxae) occurs when the acetabulum is too shallow or malformed, leading to insufficient coverage of the femoral head. It may be congenital or develop during growth. If untreated, it commonly leads to early-onset hip osteoarthritis. Treatment ranges from abduction bracing in infants to corrective pelvic osteotomy in older children and adults.
Acetabular Impingement (FAI)
Femoroacetabular impingement (FAI) is a condition in which abnormal contact occurs between the femoral head and the acetabulum during hip movement. The pincer type involves over-coverage of the femoral head by the acetabulum. This leads to pain, restricted range of motion, and over time causes cartilage damage and arthritis.
Acetabular Component in Hip Replacement
During total hip arthroplasty (THA), the acetabular component (cup) is implanted into the acetabulum. It is typically made of metal and lined with a bearing surface of polyethylene, ceramic, or metal. Correct positioning of this component is critical for the function and longevity of the prosthesis.
Diagnosis of Acetabular Pathologies
Several imaging methods are used to assess the acetabulum:
- Pelvic X-ray: Standard first-line imaging for fractures, dysplasia, and arthritis
- Computed Tomography (CT): Detailed 3D assessment of fracture patterns and bony structures
- Magnetic Resonance Imaging (MRI): Evaluation of cartilage, the acetabular labrum, and soft tissues
- Ultrasound: Particularly used in infants for the diagnosis of hip dysplasia (Graf method)
Treatment of Acetabular Conditions
Treatment depends on the specific diagnosis and its severity:
- Conservative: Physiotherapy, pain management, and weight reduction for early arthritis or mild dysplasia
- Surgical: Open reduction and internal fixation for fractures, pelvic osteotomy for dysplasia, hip arthroscopy for FAI, and total hip replacement for advanced osteoarthritis
