What Is Acidosis?
Acidosis is a condition in which the blood becomes too acidic, meaning the pH level drops below the normal range of 7.35 to 7.45. Under healthy conditions, the body maintains acid-base balance through the lungs, kidneys, and blood buffer systems. When this balance is disrupted, acidosis can develop and, if left untreated, may become life-threatening.
Causes of Acidosis
Acidosis is classified into two main types based on its origin:
- Respiratory acidosis: Caused by insufficient elimination of carbon dioxide (CO2) from the lungs, for example due to chronic obstructive pulmonary disease (COPD), respiratory failure, or severe asthma.
- Metabolic acidosis: Results from an excess of acids in the body or a loss of bicarbonate, as seen in kidney failure, diabetic ketoacidosis, lactic acidosis, or severe diarrhea.
Symptoms of Acidosis
Symptoms vary depending on the severity and cause, but may include:
- Rapid, deep breathing (known as Kussmaul breathing in metabolic acidosis)
- Nausea and vomiting
- Confusion or loss of consciousness
- Cardiac arrhythmias
- General weakness and fatigue
Diagnosis
Acidosis is primarily diagnosed using an arterial blood gas (ABG) analysis, which measures pH, carbon dioxide partial pressure (pCO2), bicarbonate (HCO3-), and base excess. Additional laboratory tests, including electrolytes, lactate, ketone bodies, and kidney function markers, as well as imaging studies, may also be used.
Acidosis Treatment
Treatment always targets the underlying cause. Symptomatic buffering alone without addressing the root condition is generally insufficient.
Treatment of Respiratory Acidosis
The primary goal is to improve ventilation:
- Supplemental oxygen and ventilatory support (non-invasive or mechanical ventilation)
- Treatment of the underlying lung condition (e.g., bronchodilators for COPD, antibiotics for pneumonia)
- Intensive care monitoring and mechanical ventilation in cases of severe respiratory failure
Treatment of Metabolic Acidosis
Therapy is tailored to the specific underlying cause:
- Diabetic ketoacidosis: Insulin administration, fluid replacement, and electrolyte correction (especially potassium)
- Lactic acidosis: Treatment of the root cause (e.g., shock management, oxygen therapy), dialysis in severe cases
- Renal acidosis: Oral sodium bicarbonate supplementation or dialysis in chronic kidney disease
- Bicarbonate infusion: Intravenous sodium bicarbonate (NaHCO3) may be used in severe metabolic acidosis, particularly when pH falls below 7.1; this approach is used selectively and remains a subject of clinical debate
- Diarrhea-induced acidosis: Fluid and electrolyte replacement, oral rehydration solutions
Dietary and Supportive Measures
In addition to medical treatment, dietary adjustments may be beneficial, particularly in mild or chronic metabolic acidosis:
- Increased intake of fruits and vegetables (alkaline-forming foods)
- Reduction of acid-forming foods (e.g., high meat consumption)
- Adequate fluid intake
When to Seek Emergency Care
Signs of severe acidosis -- such as rapid deep breathing, confusion, severe abdominal pain, loss of consciousness, or vomiting in a person with diabetes -- require immediate emergency medical attention. Severe acidosis is a medical emergency.
