What is Alendronic Acid?
Alendronic acid (also known as alendronate) belongs to a class of medications called bisphosphonates. It is used to slow down bone loss, maintain or increase bone density, and reduce the risk of bone fractures. It is a prescription-only medicine primarily used in the treatment and prevention of osteoporosis – a condition in which bones become weak and brittle over time.
Indications
Alendronic acid is indicated for:
- Postmenopausal osteoporosis in women – to prevent and treat fractures
- Osteoporosis in men – to reduce fracture risk
- Glucocorticoid-induced osteoporosis – in patients on long-term corticosteroid therapy
- Prevention of fractures of the spine, hip, and wrist in high-risk individuals
Mechanism of Action
Alendronic acid works by inhibiting osteoclasts, which are the cells responsible for breaking down bone tissue. The drug binds to the bone surface and reduces osteoclast activity, thereby slowing the rate of bone resorption. Since osteoblasts (bone-forming cells) continue to function, the balance shifts toward bone formation. This leads to increased bone mineral density and a reduced risk of fractures over time.
Dosage and Administration
Alendronic acid is available in two common dosage regimens:
- 10 mg daily – taken once every day
- 70 mg weekly – taken once a week (more commonly prescribed)
The tablet must be taken on an empty stomach, at least 30 minutes before eating or drinking anything other than plain tap water. It should be swallowed with a full glass of water (at least 200 ml), and the patient must remain upright (sitting or standing) for at least 30 minutes afterward to prevent irritation of the oesophagus.
Side Effects
Like all medications, alendronic acid can cause side effects. Common side effects include:
- Irritation or inflammation of the oesophagus and stomach
- Heartburn, abdominal pain, nausea
- Muscle, joint, or bone pain
Rare but serious side effects include:
- Osteonecrosis of the jaw – death of jawbone tissue, especially following invasive dental procedures
- Atypical femoral fractures – unusual thigh bone fractures associated with long-term use
- Severe oesophageal ulcers or inflammation
Contraindications
Alendronic acid should not be used in patients with:
- Oesophageal disorders (e.g., stricture or difficulty swallowing)
- Hypocalcaemia (low blood calcium levels)
- Severe renal impairment
- Inability to sit or stand upright for at least 30 minutes
- Known hypersensitivity to the active substance
Supportive Treatment
To support treatment with alendronic acid, adequate intake of calcium (approximately 1000–1200 mg per day) and vitamin D (approximately 800–1000 IU per day) is recommended through diet or supplementation. These nutrients are essential for bone health and help optimise the effectiveness of the medication.
