What Is an Aminocyclitol?
Aminocyclitols are organic chemical compounds featuring a saturated six-membered ring (cyclitol) with at least one amino group (-NH₂) attached. They are best known in pharmacology as the structural backbone of major antibiotic classes, most notably the aminoglycoside antibiotics. The most prominent aminocyclitol core found in these antibiotics is 2-deoxystreptamine, which forms the basis of drugs such as gentamicin, tobramycin, and amikacin.
Chemical Properties
Cyclitols are cyclic polyols, meaning ring-shaped compounds with multiple hydroxyl groups (-OH). In aminocyclitols, at least one of these hydroxyl groups is substituted by or supplemented with an amino group. This substitution gives the molecule specific biochemical properties that are critical for its interaction with bacterial ribosomes.

Medical Relevance and Mechanism of Action

This binding causes misreading of the messenger RNA (mRNA), leading to the production of faulty proteins and ultimately to bacterial cell death. This mechanism of action makes aminoglycoside antibiotics effective against a wide range of gram-negative and some gram-positive bacteria.

- Serious urinary tract infections
- Pneumonia caused by resistant pathogens
- Infections in immunocompromised patients
- Tuberculosis (streptomycin, amikacin)
Side Effects and Safety
Despite their efficacy, aminoglycoside antibiotics carry a significant side effect profile that must be considered in clinical practice:
- Nephrotoxicity: kidney damage, particularly at high doses or with prolonged use
- Ototoxicity: inner ear damage that can result in permanent hearing loss
- Neuromuscular blockade: rare, but possible at very high serum concentrations
For these reasons, regular monitoring of kidney function and serum drug levels is required during treatment, a practice known as Therapeutic Drug Monitoring (TDM).
Antibiotic Resistance
Bacteria can develop resistance to aminocyclitol-based antibiotics through several mechanisms: enzymatic inactivation of the antibiotic, modifications to the ribosomal binding site, or reduced uptake of the drug into the bacterial cell. The growing problem of antibiotic resistance makes the targeted and rational use of this drug class essential.
