Definition
The angulus subpubicus (also referred to as the subpubic angle) is the angle formed by the two inferior pubic rami (lower branches of the pubic bone) directly below the pubic symphysis. It is a fundamental component of pelvic anatomy and is routinely assessed in obstetrics and gynecology to evaluate the dimensions and shape of the female pelvis.
Anatomy and Normal Values
The subpubic angle is a key measurement used to characterize pelvic shape. In women, it typically ranges between 80° and 100°, forming an obtuse angle that facilitates the passage of the fetal head beneath the pubic arch during childbirth. In men, the angle is considerably more acute, generally ranging between 50° and 70°.
- Gynecoid pelvis (female type): subpubic angle greater than 90° -- favorable for vaginal delivery
- Android pelvis (male type): subpubic angle less than 70° -- unfavorable for vaginal delivery
- Anthropoid and platypelloid pelvis: variable angle values with differing obstetric implications
Clinical Significance in Obstetrics
In obstetrics, the angulus subpubicus is a critical parameter in clinical pelvimetry -- the measurement of the bony pelvis. A sufficiently wide subpubic angle allows the fetal head to navigate around the pubic body and pass optimally through the birth canal during delivery.
A narrow (acute) subpubic angle can contribute to cephalopelvic disproportion, a condition in which the fetal head cannot adequately fit through the maternal pelvis. This may necessitate delivery by cesarean section.
Measurement and Diagnostics
The subpubic angle can be assessed through several methods:
- Clinical examination: Manual assessment by placing both thumbs below the symphysis to estimate the angle -- a common technique used by obstetricians and midwives.
- X-ray pelvimetry: Radiographic imaging of the pelvis for precise angle measurement -- rarely used today due to radiation exposure concerns.
- MRI pelvimetry: Magnetic resonance imaging of the pelvis as a radiation-free alternative for accurate assessment of pelvic dimensions, including the subpubic angle.
- Ultrasound: Sonographic measurement used as a supplementary method, particularly during pregnancy.
Pelvic Types and Obstetric Relevance
The classification of the female pelvis according to Caldwell and Moloy distinguishes four main types, in which the subpubic angle plays a central role:
- Gynecoid pelvis: Most common form in women (~50%), wide subpubic angle, best conditions for vaginal delivery.
- Android pelvis: Acute subpubic angle, heart-shaped pelvic inlet, increased risk of obstetric complications.
- Anthropoid pelvis: Oval pelvic inlet, variable subpubic angle, vaginal delivery generally possible.
- Platypelloid pelvis: Flattened pelvis, transversely oval inlet, delivery prognosis varies depending on the angle.
