Antivenom (also known as antivenin or antivenom serum) is a biologically derived medication used to treat envenomation caused by venomous animals such as snakes, scorpions, spiders, and jellyfish. It contains antibodies that specifically bind to and neutralize the toxic components of venom, preventing serious organ damage or death.
Production and Mechanism of Action
Antivenom is produced by immunizing animals - typically horses or sheep - with gradually increasing, sub-lethal doses of the target venom. The immune system of these animals responds by producing specific antibodies against the venom components. These antibodies are then extracted from the blood, purified, and processed into a medical product.
When administered to a patient, the antibodies in the antivenom circulate through the bloodstream and bind to the venom molecules (antigens). This antigen-antibody complex is then cleared by the body, preventing further toxic damage to organs, the nervous system, or the blood clotting system.
Indications and Uses
- Snakebites: Antivenom is the primary and most effective treatment for severe snakebite envenomation, including bites from cobras, mambas, rattlesnakes, and vipers.
- Scorpion stings: Particularly important for stings from dangerous species such as the fat-tailed scorpion.
- Spider bites: Used in cases of bites from Black Widow spiders or the Brown Recluse spider.
- Jellyfish stings: Specific antivenoms exist for highly venomous species such as the Box jellyfish (sea wasp).
Administration and Dosage
Antivenom is most commonly administered intravenously (directly into a vein), as this method ensures the fastest and most effective distribution throughout the body. Intramuscular injection may be used in some settings. The appropriate dose depends on the severity of the envenomation, the species involved, and the patient's clinical condition. Prompt administration is critical to prevent irreversible damage.
Side Effects and Risks
Because antivenom is derived from animal proteins, it carries the risk of triggering immune reactions. Potential side effects include:
- Skin reactions such as itching or rash
- Fever and chills
- Nausea and vomiting
- Serum sickness (a delayed immune reaction occurring days after administration)
- In rare cases: anaphylaxis (a severe, life-threatening allergic reaction)
For these reasons, antivenom should always be administered in a medical facility equipped with emergency resuscitation equipment.
Global Availability and Importance
Snakebite envenomation is recognized as a major global public health problem. The World Health Organization (WHO) estimates that up to 2.7 million people are envenomated by snakes each year, resulting in approximately 81,000 to 138,000 deaths. Access to antivenom remains severely limited in many parts of Africa, Asia, and Latin America, contributing to preventable deaths. The WHO has therefore included antivenom on the List of Essential Medicines.
