Definition
Asteroid bodies are small, star-shaped inclusion bodies found within multinucleated giant cells – such as Langhans giant cells or foreign body giant cells. Their name derives from their characteristic star-like, radiating appearance under the microscope. They consist of a central core from which fine, ray-like projections extend outward.
Occurrence and Clinical Significance
Asteroid bodies are not specific to a single disease, but they are most frequently observed in the following conditions:
- Sarcoidosis: The most common association; they are a characteristic histological feature of non-caseating granulomas in this disease.
- Foreign body granulomas: Reactions to foreign materials such as silicates, metal, or plastic particles.
- Mycoses (fungal infections): Certain fungal infections such as sporotrichosis or lobomycosis can produce asteroid bodies.
- Tuberculosis: Occasionally observed, though less frequently than in sarcoidosis.
- Berylliosis: A lung disease caused by beryllium dust inhalation.
Structure and Composition
Ultrastructurally, asteroid bodies consist of a network of collagen fibers alongside cellular lipid and protein components. The star-shaped projections arise from the radial arrangement of filaments within the cytoplasm of the giant cells. They are not living organisms but rather cellular reaction products.
Diagnosis
Asteroid bodies are typically identified through a tissue biopsy, which is subsequently examined histologically under a microscope. They are clearly visible with hematoxylin and eosin (H&E) staining. The presence of asteroid bodies alone does not establish a diagnosis and must always be evaluated within the broader clinical context.
Typical Diagnostic Methods
- Light microscopy with H&E staining
- Electron microscopy for detailed ultrastructural analysis
- Immunohistochemical staining for further characterization
Differentiation from Similar Structures
Asteroid bodies must be distinguished from other intracellular inclusion bodies, such as:
- Schaumann bodies: Calcified, lamellar inclusion bodies also typical of sarcoidosis.
- Hamazaki-Wesenberg bodies: Yellowish-brown granules found particularly in lymph nodes.
Clinical Relevance
The presence of asteroid bodies in a biopsy specimen indicates a granulomatous inflammatory reaction and can provide the clinician with important diagnostic clues. Particularly in cases of unexplained pulmonary disease or lymphadenopathy, their identification may support a suspected diagnosis of sarcoidosis. Treatment is not directed at the asteroid bodies themselves, but rather at the underlying disease.
