What is Pansinusitis?
Pansinusitis refers to the simultaneous inflammation of all paranasal sinuses – including the maxillary sinuses, frontal sinuses, ethmoid cells, and the sphenoid sinus. The term derives from the Greek word pan (all) and sinusitis (inflammation of the paranasal sinuses). It represents the most severe form of sinusitis and can occur in both acute and chronic forms.
Causes
Pansinusitis often develops as a complication of untreated or incompletely resolved sinusitis. Common causes include:
- Viral infections (e.g., common cold viruses, influenza)
- Bacterial infections (e.g., caused by Streptococcus pneumoniae or Haemophilus influenzae)
- Allergies (allergic rhinitis as a trigger)
- Anatomical variations such as a deviated nasal septum
- Nasal polyps obstructing sinus drainage
- Weakened immune system, e.g., in diabetes or patients on immunosuppressive therapy
Symptoms
The symptoms of pansinusitis are typically more severe than those of ordinary sinusitis, since all paranasal sinuses are involved:
- Intense pressure and pain across the entire face (forehead, cheeks, between the eyes)
- Nasal congestion and impaired nasal breathing
- Purulent or mucous nasal discharge
- Reduced or absent sense of smell and taste
- Headaches that worsen when bending forward
- Fever (especially in the acute form)
- General malaise and fatigue
- Tenderness when pressing on the sinus areas
Diagnosis
Diagnosis is established by a physician using several methods:
- Medical history and physical examination: Assessment of symptoms and palpation of sinus areas
- Nasal endoscopy: Direct visualization of the nasal cavity and mucous membranes using a thin endoscope
- Imaging: A CT scan of the paranasal sinuses (computed tomography) is the gold standard and reliably shows all affected sinuses and potential complications
- Swabs and laboratory tests: Used when bacterial infection is suspected or to identify the causative organism
Treatment
Conservative Treatment
Treatment depends on the underlying cause and severity of the condition:
- Decongestant nasal sprays or drops to improve sinus ventilation
- Corticosteroid nasal sprays to reduce mucosal swelling
- Antibiotics in confirmed bacterial infections (e.g., amoxicillin)
- Saline nasal irrigation to cleanse and moisturize the nasal passages
- Antihistamines if an allergic cause is identified
- Adequate fluid intake and rest
Surgical Treatment
If pansinusitis does not respond to conservative therapy or a chronic form is present, surgical intervention may be necessary. Functional endoscopic sinus surgery (FESS) is the standard procedure. It involves removing polyps, widening narrowed drainage pathways, and restoring ventilation of the paranasal sinuses.
Complications
If left untreated, pansinusitis can lead to serious complications, including:
- Spread of infection to the eye socket (orbital complications)
- Inflammation of the meninges (meningitis)
- Brain abscess
- Osteomyelitis of adjacent bones
