- Patellar reflex (knee-jerk reflex): Tapping the patellar tendon just below the kneecap causes extension of the lower leg.
- Achilles reflex: Tapping the Achilles tendon causes plantarflexion of the foot.
- Biceps reflex: Tapping the biceps tendon in the antecubital fossa causes flexion of the forearm.

Several models of percussion hammers exist, differing in shape, size, and intended use:
- Taylor hammer: Triangular rubber head; widely used in general practice and neurology.
- Babinski hammer: Round rubber head on a thin handle; frequently used in neurology.
- Queen Square hammer: Round head on a long, flexible handle; particularly suited for testing the patellar reflex.
- Troemner hammer: Compact design; often used in pediatrics.
Clinical Significance
Reflex testing with a percussion hammer provides valuable information about the condition of the nervous system. Altered reflexes can indicate various neurological disorders:
- Diminished or absent reflexes (hyporeflexia / areflexia): Suggest peripheral nerve damage, such as in polyneuropathy or herniated discs.
- Exaggerated reflexes (hyperreflexia): Suggest damage to the central nervous system (CNS), such as in multiple sclerosis, stroke, or spinal cord disease.
- Pathological reflexes: Certain abnormal reflex responses (e.g., Babinski sign) indicate lesions of the upper motor neuron.
How the Examination is Performed
Reflex testing is performed with the patient in a relaxed position. The physician strikes the appropriate tendon with the percussion hammer and observes the reflex response. The intensity of the response is rated on a standardized scale (0 = no reflex, up to 4 = markedly hyperactive with clonus). Experience and practice are essential for accurate interpretation of the results.
