What is the Periphericus?
The term Periphericus (Latin: nervus periphericus) refers to a peripheral nerve, meaning a nerve that belongs to the peripheral nervous system (PNS). The peripheral nervous system comprises all nerve fibers and nerve structures located outside the central nervous system (CNS) – that is, outside the brain and spinal cord. These nerves connect the CNS to the various organs, muscles, glands, and skin throughout the body.
Structure and Function
A peripheral nerve consists of bundled nerve fibers (axons) enclosed by connective tissue. Several fiber types can be distinguished:
- Motor fibers: Transmit movement commands from the brain to the muscles.
- Sensory fibers: Carry sensations such as pain, temperature, touch, and pressure from the body to the brain.
- Autonomic fibers: Control involuntary bodily functions such as heart rate, digestion, and glandular secretion.
The peripheral nervous system is divided into the somatic nervous system (voluntary control of movement and sensory perception) and the autonomic nervous system (involuntary control of internal organs).
Clinical Relevance
Diseases or damage affecting the Periphericus are referred to as peripheral neuropathy or polyneuropathy. Common causes include:
- Diabetes mellitus (diabetic polyneuropathy)
- Vitamin deficiencies (e.g., vitamin B12)
- Infectious diseases (e.g., Lyme disease, herpes zoster)
- Autoimmune conditions
- Traumatic injuries (e.g., nerve compression or severance)
- Toxic exposure (e.g., alcohol, heavy metals)
Symptoms of Nerve Damage
Damage to the Periphericus can cause a wide range of complaints:
- Numbness and tingling (paresthesias)
- Burning or stabbing pain
- Muscle weakness or paralysis
- Reduced or absent reflexes
- Autonomic symptoms such as sweating disorders or circulatory problems
Diagnosis
Several diagnostic procedures are available to evaluate the Periphericus:
- Electroneurography (ENG): Measurement of nerve conduction velocity
- Electromyography (EMG): Assessment of muscle electrical activity
- Laboratory tests: Blood work to identify underlying causes
- Nerve biopsy: Tissue sampling in unclear cases
- Imaging (MRI): Visualization of nerve compression or tumors
Treatment
Treatment is directed at the underlying cause and may include:
- Blood glucose optimization in diabetic neuropathy
- Vitamin supplementation in confirmed deficiency states
- Physiotherapy to maintain muscle function
- Pain management (e.g., anticonvulsants, antidepressants, or analgesics)
- Surgical intervention for nerve compression or injury
