What is Pipamperone?
Pipamperone is a low-potency antipsychotic drug belonging to the chemical class of butyrophenones. It is used primarily in geriatric medicine, psychiatry, and sleep medicine. Compared to other antipsychotics, pipamperone has a weaker effect on the motor system, making it generally well tolerated and particularly suitable for elderly patients.
Indications
Pipamperone is mainly used in the following conditions:
- Sleep disorders: difficulties falling and staying asleep, especially in elderly patients
- Agitation and restlessness: for example, in dementia or other psychiatric conditions
- Behavioral disorders: in children with intellectual disabilities or pervasive developmental disorders
- Psychomotor agitation: in the context of various psychiatric conditions
Mechanism of Action
Pipamperone exerts its effects primarily through blockade of dopamine receptors (particularly D4 receptors) and inhibition of serotonin receptors (5-HT2A receptors) in the brain. This dual action results in a reduction of agitation, anxiety, and restlessness. Additionally, pipamperone has a mild antihistaminergic component that contributes to its sedative effect.
Compared to high-potency antipsychotics such as haloperidol, the dopamine-blocking effect of pipamperone is considerably weaker, which reduces the risk of extrapyramidal side effects (movement disorders).
Dosage
The dosage of pipamperone depends on the indication, the age, and the general health status of the patient. Typical daily doses range from 40 mg to 360 mg, divided into several administrations. In elderly patients, treatment is usually initiated at a low dose and increased gradually as needed. The exact dosage must always be determined individually by a physician.
Side Effects
Like all medications, pipamperone can cause side effects. Common and well-known side effects include:
- Sedation and drowsiness: especially at the beginning of treatment
- Orthostatic hypotension: a drop in blood pressure upon standing, particularly in elderly patients
- Extrapyramidal symptoms: movement disorders such as tremor or rigidity (less common than with high-potency antipsychotics)
- Weight gain: possible with prolonged use
- Cardiac arrhythmias: rare cases of QT interval prolongation on ECG
- Dry mouth and constipation: due to anticholinergic effects
Contraindications and Drug Interactions
Pipamperone should not be used in patients with known hypersensitivity to the active substance, in cases of severe central nervous system depression caused by alcohol or other sedative substances, or in patients with certain cardiac conditions involving prolonged QT interval.
Important drug interactions exist with:
- Other centrally acting depressants (hypnotics, anxiolytics, alcohol)
- Medications that prolong the QT interval
- Antihypertensive agents, as pipamperone may potentiate their blood pressure-lowering effect
Special Considerations
In elderly patients with dementia, particular caution is required when using antipsychotics, as this population has an increased risk of stroke and other serious adverse events. Pipamperone should only be used in this group after careful risk-benefit assessment and under medical supervision. Pregnant and breastfeeding women should only take this medication after consulting a physician.
