What is Piritramide?
Piritramide is a synthetic opioid analgesic classified among the strong-potency pain relievers used in clinical medicine for the treatment of severe to very severe pain. It belongs to the group of opioid analgesics and is subject to strict narcotic control regulations in many countries. In Germany and several other European countries, piritramide is marketed under the trade name Dipidolor and is used exclusively in hospital settings under medical supervision.
Indications
Piritramide is indicated for:
- Postoperative pain: It is one of the most commonly used opioids in recovery rooms following surgical procedures.
- Severe acute pain: Including pain associated with cancer, trauma, or other serious medical conditions.
- Patient-controlled analgesia (PCA): Patients can self-administer pre-set doses on demand via a monitored pump system.
Mechanism of Action
Piritramide exerts its analgesic effect by binding to mu-opioid receptors in the central and peripheral nervous systems. These receptors are part of the body's endogenous pain-modulating system. Activation of mu-opioid receptors inhibits the transmission of pain signals in the brain and spinal cord, resulting in significant pain relief. The analgesic potency of piritramide is broadly comparable to that of morphine, though it is generally considered to be approximately 0.7 times as potent.
Dosage and Administration
Piritramide is administered exclusively by parenteral routes, typically intravenously (IV) or intramuscularly (IM). No oral formulation is available.
- Adults: The usual single dose is 0.1--0.2 mg per kilogram of body weight, adjusted according to pain severity and clinical context.
- PCA pump: Typical bolus doses of 1.5--2 mg with a lockout interval of 5--10 minutes are used.
- Children and adolescents: Use is possible but requires careful dose adjustment and close monitoring.
The exact dosage is always determined individually by the treating medical team.
Side Effects
As with all opioids, piritramide can cause a range of side effects:
- Common: Nausea, vomiting, drowsiness (sedation), dizziness
- Common: Constipation, itching (pruritus)
- Rare but serious: Respiratory depression (slowed or shallow breathing) -- the most dangerous side effect of all opioids
- Other: Hypotension (low blood pressure), dry mouth, urinary retention, muscle rigidity at high doses
Due to the risk of respiratory depression, piritramide must only be used under strict medical supervision with resuscitation equipment available. Naloxone is the specific antidote in cases of opioid overdose.
Contraindications
Piritramide should not be used or should be used with extreme caution in:
- Known hypersensitivity to piritramide or other opioids
- Severe respiratory depression or obstructive airway disease
- Concomitant use of monoamine oxidase (MAO) inhibitors
- Severe hepatic or renal impairment
- Inadequately treated hypothyroidism (underactive thyroid)
- Pregnancy and breastfeeding (only after careful benefit-risk assessment)
Drug Interactions
The effects of piritramide can be enhanced by other centrally depressant substances such as benzodiazepines, alcohol, general anesthetics, and antihistamines, increasing the risk of respiratory depression. Concurrent use with MAO inhibitors is contraindicated.
Dependence Potential
As an opioid, piritramide carries a risk of dependence and misuse. However, when used as intended in a clinical setting for short-term pain management, the risk is considered low. Its classification as a controlled narcotic ensures strict regulations regarding storage, prescription, and documentation are followed.
