Definition
The plica rectouterina (also written plica recto-uterina) is a paired fold of the peritoneum located in the female pelvis. It extends bilaterally from the posterior surface of the uterus to the rectum and forms the lateral boundaries of the rectouterine pouch, commonly known as the pouch of Douglas (excavatio rectouterina) -- the deepest recess of the female peritoneal cavity.
Anatomy and Location
The plica rectouterina is situated in the posterior compartment of the lesser pelvis. Together with its counterpart on the opposite side, it forms a crescent-shaped boundary around the pouch of Douglas. Within the fold run smooth muscle fibers and connective tissue strands known as the uterosacral ligaments (ligamenta rectouterina). These ligaments play an important role in maintaining the normal position and support of the uterus within the pelvis.
Clinical Significance
The plica rectouterina and the adjacent pouch of Douglas are clinically important for several reasons:
- Endometriosis: Endometriotic lesions frequently implant on the uterosacral ligaments and the plica rectouterina. This can cause severe menstrual pain (dysmenorrhea), pain during intercourse (dyspareunia), and chronic pelvic pain.
- Culdocentesis: The pouch of Douglas is accessible through the posterior vaginal wall. When free fluid is suspected -- such as blood in the case of an ectopic pregnancy or pus in pelvic inflammatory disease -- a needle aspiration procedure known as culdocentesis may be performed for diagnostic purposes.
- Pelvic Infections: In cases of pelvic inflammatory disease (PID), pus may collect in the pouch of Douglas, forming a Douglas abscess.
- Tumor Spread: Malignancies of the ovaries, uterus, or rectum can spread along the plica rectouterina and form deposits within the pouch of Douglas.
Diagnosis
The plica rectouterina and the pouch of Douglas can be assessed through several diagnostic methods:
- Vaginal examination: Nodularity or tenderness along the uterosacral ligaments and the posterior cul-de-sac can be detected by bimanual or rectovaginal palpation.
- Transvaginal ultrasound: Free fluid or an abscess in the pouch of Douglas can be identified using ultrasound imaging.
- Pelvic MRI: Magnetic resonance imaging provides detailed visualization of the plica rectouterina and is particularly useful for identifying endometriotic lesions or tumor infiltration.
- Laparoscopy: Direct visualization via minimally invasive surgery remains the gold standard for diagnosing endometriosis affecting the plica rectouterina and uterosacral ligaments.
