What Is the PSA Test?
The PSA test is a blood test that measures the level of prostate-specific antigen (PSA) in the blood. PSA is a protein produced exclusively by the prostate gland. Elevated PSA levels may indicate various prostate conditions, including prostate cancer (prostate carcinoma), a benign enlarged prostate (benign prostatic hyperplasia, BPH), or prostate inflammation (prostatitis).
How Does the Test Work?
A small blood sample is drawn from a vein in the arm. The sample is then analyzed in a laboratory to determine the concentration of PSA protein in the blood serum. Results are expressed in nanograms per milliliter (ng/mL).
Reference Values and Interpretation
There is no universally agreed-upon threshold that definitively distinguishes a healthy prostate from a diseased one. However, the following general reference ranges are commonly used:
- Below 4 ng/mL: Generally considered normal, though prostate cancer cannot be entirely ruled out.
- 4 to 10 ng/mL: A grey zone; the risk of prostate cancer is elevated and further investigation is recommended.
- Above 10 ng/mL: A significantly elevated level associated with a substantially increased risk of prostate cancer.
It is important to note that PSA levels can also be affected by factors such as age, prostate volume, physical activity, and certain medications.
When Is the PSA Test Used?
The PSA test is used in a variety of clinical situations:
- Early detection: As a screening tool for prostate cancer, particularly in men aged 50 and older, or from age 45 in those with an elevated family risk.
- Monitoring treatment: To track the effectiveness of prostate cancer treatment (e.g., after surgery or radiation therapy).
- Diagnostic support: In cases of symptoms such as frequent urination, painful urination, or blood in the urine.
- Recurrence detection: To identify a possible relapse following cancer treatment.
Benefits and Limitations of the PSA Test

Limitations and Risks
- False-positive results: Elevated PSA levels can result from benign conditions, potentially leading to unnecessary further testing or procedures.
- False-negative results: A normal PSA level does not completely exclude the presence of prostate cancer.
- Overdiagnosis: Some detected prostate cancers may never have caused clinical symptoms and may not require treatment.
What Happens After an Elevated PSA Result?
If the PSA test result is abnormal, a physician will typically recommend further diagnostic steps:
- Digital rectal examination (DRE): The doctor manually examines the prostate through the rectum to detect any hardening or irregularities.
- Transrectal ultrasound (TRUS): An ultrasound examination of the prostate conducted via the rectum.
- Prostate biopsy: Tissue samples are taken from the prostate and examined under a microscope.
- Prostate MRI: Magnetic resonance imaging provides detailed images of the prostate tissue to assess any abnormalities.
